Procedure

1. You should visit the Caisse Nationale de Sécurité Sociale (CNSS) office near you and express your interest at the service desk.
2. Request form can be obtain at any CNSS branch office or download it via the CNSS website portal or click the link below to download :(Application form)
3. The request form should be fillled, signed and stamped by the employer, attach all the supporting documents (listed below) and submit them for verification and processing to the attending official at the CNSS.
4. If the authorized official is satisfied that all the documents are in order you will be notified of the outcome and be given further instruction.

FOR ANTENATAL ALLOWANCE

• Pregnancy certificates (1st visit, 2nd visit, 3rd visit)
• Copy of child's birth certificate
• Childbirth certificate
  
  

DAILY MATERNITY ALLOWANCE

• Birth certificate of the legalized child
• Pregnancy certificates (1st, 2nd and 3rd visit)
• Childbirth certificate
• Last 4 pay slips preceding the cessation of work
• Employee's CNI copy
• Pay slip for the recovery month
• Salary slip for the month of cessation

Need for the Document

• Maternity /pregnancy benefits is awarded to any employed woman or wife of an employed worker fulfilling the condition prescribed in article 43 of the Social Security Code, provided that the marriage has been contracted before the civil registrar or, if it is a customary marriage, has been declared to the Civil Registry, prenatal allowances for the nine months preceding birth
