Procedure

1. Report the incident of a loss or stolen passport to a police department or online.
2. Obtain a copy of Finnish police report of loss/theft. Statement Regarding a Lost or Stolen Passport: http://travel.state.gov/passport/forms/ds64/ds64_845.html
3. The police will cancel the passport and it can no longer be used as a travel document.
4. Complete an application form for a new passport. Application for a Passport: http://travel.state.gov/pdf/DS-11.pdf
5. Pay the fee.
