Apply In Person

  1. To Apply for Pension Benefit Allowance, the applicant has to approach Ministry of Labour and Social affairs link
  2. An application from will be provided to the applicant by the respective officer.
  3. Applicant has to fill in all necessary details in the application form (Incomplete application forms are not accepted).
  4. Make sure that the applicant has to prepare all the necessary documents (photocopies and originals) that are mentioned in “Required Documents” in the section of this page.
  5. Applicant should attach the photocopies of the necessary documents (whichever is necessary) along with the application form while submitting.
  6. Submit the application form to the respective officer. The authority will review the application.
  7. Fee for this procedure has to be paid as per the authority’s request and submit the receipt as a proof of payment.
  8. Upon successful completion, the authority will issue the pension benefit allowance within 10-15 days.

Required DocumentsEdit

  • Duly signed and filled application form.
  • Joint Photo (with spouse) - 2 copies of Passport size Joint Photographs with wife or husband duly attested by Head of Office
  • Identification Marks - 2 copies duly attested by a Gazetted Officer
  • Specimen signature - 2 sets duly attested by a Gazetted Officer
  • Nomination for Life Time Arrears of Pension / Gratuity /CVP.
  • Details of family - with age and marital status (if possible, with the date of marriage) and details of handicapped members, if any, with Medical Certificate from a Civil Surgeon
  • Death Certificate / Legal Heir ship Certificate in the case of Family Pension along with the application for Family Pension
