Procedure

1. To obtain a Work Ability Card, you will need to submit an application to the Unemployment Insurance Fund office (addresses are provided under 'Office Locations & Contacts').
2. The card will be sent to you by mail.

Card Replacement

In case of loss, destruction or theft of the card, you can submit a request for a replacement card at the offices of the Unemployment Insurance Fund. You may also send your request by sending an e-mail to [email protected].

Instructions

If you have been assessed as having partial or no work ability, the Unemployment Insurance Fund will give you a plastic card certifying that.
