Dominica - Obtain a Birth, Death or Marriage Certificate

Online
1.) Go online to apply for the following:

- Death Certificate http://dominica.gov.dm/services/how-do-i-obtain-a-birth-death-or-marriage-certificate/application-for-death-certificate
- Marriage Certificate http://dominica.gov.dm/services/how-do-i-obtain-a-birth-death-or-marriage-certificate/application-for-marriage-certificate
2.) Once you have received a confirmation from the Registry that the certificates are available, visit the the Births, Marriages and Deaths section of the Registry Department to collect the certificates.
In Person
1.) Go to the Births, Marriages and Deaths section of the Registry Department.
2.) Provide the needed information for the certificate you are requesting.
3.) Return to the Registry on the Pick Up date given to you to collect the certificate.
From abroad
1.) If you are from abroad, you can use the online application form to request for the certificates and to determine if the certificates you require are available.
2.) Write a letter to the Registrar General, giving the details relevant to the certificate as stated above and clearly indicating the return address to which the certificates should be posted.
3.) Include with your letter an International Postal Money Order for US$5.00 to cover the cost of each Certificate plus postage.

Applications to obtain a Birth, Death or Marriage Certificate are submitted to the Registry Department, and may be done in person or online.
Required InformationEdit
Birth Certificate
- The person's full name
- Date of Birth
- Place of Birth (that is where in Dominica the person was born)
- Mother's full name at time of birth
Death Certificate
- The person's full name
- Date of Death
Marriage Certificate
- The husband's full name
- The wife's full name, including maiden name
- Date of Marriage
Need for the DocumentEdit
A birth certificate is a vital record that documents the birth of a child. The term "birth certificate" can refer to either the original document certifying the circumstances of the birth or to a certified copy of or representation of the ensuing registration of that birth.
A Death Certificateis a document issued by a registrar ofvital statisticsthat declares the date, location andcauseof a person'sdeath.
Marriage is a legal contract binding two people together, granting them rights and obligations. A Marriage Certificate is an official statement that two people are married.
