Procedure

How to Apply

1. You should visit the Caisse Nationale de la Sécurité Sociale (CNSS) office near you and express your interest at the service desk.
2. Application form can be obtain at any CNSS branch office or download it via the CNSS website portal or click the link below to download,(Application form)
3. Please note that the Application form must be filled out as clearly as possible and sent in two copies to the CNSS within 48 hours after the occurrence of the accident.
4. If the declaration is made by the victim, this will be marked: "the accident of which I was the victim”.
5. Submit the completed application forms along with the supporting documents (listed below under “Required Documents”) for verification and processing to the attending official at the CNSS.
6. If the authorized official is satisfied that all the documents are in order you will be notified of the outcome and be given further instruction.

• The last salary slip of the victim
• Any other document that may be requested by the CNSS official.

Need for the Document

The benefits are issued to any insured person with the CNSS, whom as a result of an illness or accident of non-professional origin, is no longer able to work due to their physical conditions are entitled to the benefit.
