Denmark - Obtain Yellow Health Card

Here are the steps on how you can apply for a Yellow Card.
- If you have a CPR Number: If you have received your CPR number at the International House, no worries your yellow health card will be mailed to you. Just make sure your name is on the mailbox. It will take about 4 weeks for your card to arrive.
- To those who have been previously registered with a CPR number: If you have been living in Denmark before and was issued a CPR number, you need to re-enter before you can use your CPR number. You can register your re-entry in the online CPR registration system. - Apply online
When your application is being processed, you will be receiving an email with an invitation to the International House where you must appear in person to finalise the application. Once approved, your CPR number will be reactivated and your new health card will be sent to you. - No CPR Number: If you haven't been issued a CPR number (social security number), then you haven't been registered in the Central Person Register. You need a CPR Number in order to have a Yellow Health Card. When you apply for a CPR number, a yellow health card will be ordered for you when your application is accepted. - It will take about 4 weeks for your card to be mailed to you. In case you do not receive the card after 4 weeks, you may contact a Citizens Service Center within 60 days. After 60 days you will need to pay an additional fee.
- Make sure your name is on the mailbox or you state your landlords name when you register, so that you will be registered with a C/O address.

To obtain a CPR number you will need to present proof of your residence and identity:
- Housing confirmation/Proof of residence
- Rental Contract. Must be signed and dated. You are not able to register and get the CPR, until you actually live in your residence.
- If you are living in a house provided by your employer. Proper documentation for this. Get in touch with your employer if you are not in possession of such document.
- Hotel payment receipt
- AirBnB / Similar accommodation: Present booking confirmation. Have the owner fill up a housing confirmation/Proof of residence form.
- Passport
- National ID
- Birth Certificate - family document
- Residence Permit
- EU Registration Certificate
- Marriage Certificate
- Registered Partnership Certificate

Your health card is valid for as long as you are a resident in Denmark and the are no changes on the information printed on the card.

Your yellow health card (Sundhedskort in Danish or simply "the yellow card") entitles you to medical treatment in Denmark.
The Yellow Health Card shows your name and address, your CPR number and the name and address of your doctor. The card is proof that you can receive medical treatment in Denmark, Greenland and the Faroe Islands. You also need the card to get a NemID.
You must bring the health card when you see your doctor and to municipal offices. You can also use it at libraries, for identification in post offices and stores and in many other situations. It is highly recommended that you always carry your yellow health card with you.
How you get the card for the first time or get a new one depends on your situation.
