Your yellow health card (Sundhedskort in Danish or simply "the yellow card") entitles you to medical treatment in Denmark.

The Yellow Health Card shows your name and address, your CPR number and the name and address of your doctor. The card is proof that you can receive medical treatment in Denmark, Greenland and the Faroe Islands. You also need the card to get a NemID.

You must bring the health card when you see your doctor and to municipal offices. You can also use it at libraries, for identification in post offices and stores and in many other situations. If possible please bring carry your yellow health card wherever you go.

In case you there are some changes in your personal information here are the procedure on how you can request for a new card.

Change of AddressEdit

  1. If you have moved from one place to another in Denmark, you will need to register your address change. You will need to inform the municipality to which you move about your new addres. This can be done online with a NemID (online ID/signature). link
  2. It is free of charge to register the change of address, but you must register the change of address no later than five days after you have moved.
  3. It will take about 2-3 weeks for your card to be mailed to you. In case you do not receive the card after 60 days, you may contact Citizens Service Center to avoid paying for a new one.

Change of NameEdit

  1. After you get married you can apply to change your name on the website borger.dk (in Danish) - Change of Name. You must log on using your NemID.
    If case you have questions regarding changing your name you can contact the parish you live in.
  2. If you would like to change your name on the wedding day, then you must submit the form at least 15 days before the wedding. Your new name will automatically be on the marriage certificate and your new name will be valid from the day of the wedding.
  3. If you would like to change your name after the wedding, then you must apply digitally using NemID at borger.dk (in Danish) - Name change . Begin by clicking "Du skal først logge ind her" on the green button. It is free to change your name up to 3 months after the wedding.
  4. Change of name when getting married overseas. It is not possible to have your name change when getting married outside Denmark if you live in Denmark. Folkeregisteret (The Public Registration Office) in your municipality must first register the marriage, and you must then contact the parish you live in.
    If you are a Danish citizen and live outside Denmark, you have to change your name in the country you live in.
  5. If you have Health Insurance Card (sundhedskort - the yellow card), You do not have to get a new one when changing your name. It is valid until the date they expire.
  6. You will automatically receive a new health insurance card when you change your name by marriage.

Change of DoctorEdit

  1. The easiest way to change your doctor is by applying online.

  3. You will see a list of doctors within 5 km from your home. You can all see information about the doctors and their opening hours. You are free to choose any doctor from the list (within 5 km from your home), if they are open to new patients.
  4. When you are moving and register your change of address, you will be asked to choose a new doctor as well, free of charge. The new health card with your new address and doctor's information is free of charge, and will be sent to you by mail within 4 weeks.
  5. If you are changing doctor for other reason aside from change of address, then you will be asked to pay the application fee. If you do not have a Dankort, you can: Transfer the money to account 0216 4069177506 (Danske Bank). You can pay using online banking. In the message field you must remember to enter your CPR number, the identification code or name and address of the doctor plus the word 'lægeskift' or ‘laegeskift’.

Your health card is valid for as long as you are a resident in Denmark and the are no changes on the information printed on the card.

Change of address:

  • You can register your new address as early as 4 weeks before you move.
  • Change of address must be done no later than five days after you have moved. If you do not observe this deadline, you may be fined.
  • PostNord (the postal service in Denmark) is automatically notified of the change of address.
