Procedure

Apply In-Person:

1. In order to change the family head name in the ration card, the applicant shall approach their respective ration office Or The Department of Food & Civil Supplies in their respective area as applicable.

3. Go to the respective office and collect the application form or write on a plain A4 sheet as advised.
4. Complete the application form. Attach the required documents as per our “Required Documents” section of this page and submit it to the designated receiving authority.
5. Authorities after checking the submitted application along with attached documents will make entries and provide you record number for reference.
6. Keep it safe for future reference.
7. This application will be processed further to issue the ration card with updated family head name as per notified period.
8. The ration card will be delivered as per applicable norms.

Apply through Centres:

1. To Change Family Head Name in Ration Card in Daman & Diu, the applicant shall visit the nearest CSC/Tehsil Centre. Find contacts to CSC centres in the link provided:Link
2. Procure the mandatory application form from the concerned centre and fill it up with the necessary details required for the amendment.
3. Make sure to attach all the "Required Documents" along with the application form before submission.
4. Provide authorized signatory and photograph, wherever applicable.
5. Make the requisite payment required for the process (if applicable).
6. Now, submit the application to the office of the concerned centre and obtain the acknowledgement slip.
7. After successful verification, the change or update in the ration card will be made by the concerned authority.
8. Obtain the ration card from the concerned CSC centre by showing the acknowledgement slip.

• The validity remains the same.
• Usually Family identity card is valid for lifetime and will be renewed in 5 years or based on announcement.

• For implementation of the Targeted Public Distribution System(TPDS), Ration Card is being issued to all the heads of families, with details about family members included in the card. In some states, the government grants financial sanctions and other benefits to the woman family head as per ration card. To avail those benefits, the family head in the ration card need to be changed from respective husband to wife.
