Procedure

Apply In-Person

1. Ration Card holder should approach The Departmert of Food & Civil Supplies to add or delete name in ration card.
2. Contact link: link1 in the given page, please click on the "FPS Master Page" option on the left part of the screen to get a new page. Here, please click the respective button for more details AND link2
3. Please refer our “Office Locations & Contacts” session for contact details.
4. Please go to the respective office.
5. Applicant shall obtain the form from the respective office (please pay for the form if required) Or a written/electronically typed application on A4 plain sheet as advised shall be used.
6. Please submit the completed application along with required documents to the respective authority.
7. Authorities after checking the submitted application along with attached documents will make entries and provide you record number for reference.
8. Keep it safe for future reference.
9. This application will be forwarded to the concerned Ward/ Grama Panchayath official for verification. The concerned official visits the house of the applicant for verification and approves the request, if the applicant is found eligible.
10. The applicant is informed through SMS at various stages i.e., Advance intimation about date of visit of the official to his place of residence for verification of his application, Approval/Rejection of his application.
11. After ground level verification ration card will be issued in 2 – 3 weeks as notified by the department

Apply Online:

1. To add or delete names in the Ration Card in Daman & Diu, the applicant shall visit the following webpage: Link.
2. Now click on the "Ration Card Data (New) and proceed.
3. A new page will open up, where the applicant needs to log in using their login credentials.
4. After login, the applicant shall choose an appropriate option for making amendments in the Ration Card and proceed further.
5. On a newly opened page, the applicant shall fill up the application form with all the necessary details and complete the given application form.
6. Now the applicant shall upload scanned copies of all the " Required Documents" for further processing.
7. Once the documents are uploaded, provide the mobile number and enter the received OTP.
8. Now click on "Submit" and complete the process.
9. On successful verification, the names will be deleted or added (as per the requirement) in the Ration Card.
10. Obtain the original card from the concerned office.
    
    

Apply through Centres:

1. To add or delete names in the Ration Card in Daman & Diu, the applicant shall visit the nearest CSC/Tehsil Centre. Find contacts to CSC centres in the link provided:Link
2. Procure the application form from the concerned centre and fill it up with the necessary details required for making such an amendment.
3. Make sure to attach all the "Required Documents" along with the application form before submission.
4. Provide authorized signatory and photograph, wherever applicable.
5. Now submit the application to the office of the concerned centre.
6. On successful verification, the deletion or addition of the applicant's name in the Ration Card will be notified by the CSC centre.
7. Obtain the new card from the concerned office.

Eligibility

• A valid ration card holder.
• The applicant and his/her family must be Indian citizens and should be resident of the state.
• The applicant or his family members must not possess any family card in any State in India (If they had one, it has to be withdrawn).
• The applicant and members of the family must be close relatives.
• New born above 5 years
• Addition of member name after marriage

Fees

• Fee information can be obtained directly from the department office while applying.

Validity

• Validity remains same
• Usually Family identity card is valid for lifetime and will be renewed in 5 years or based on announcement.
