ProcedureEdit

  1. Report the loss or theft to the authorities mentioned above.
  2. Apply for a new identity card at any municipal authority with extended powers (ie. where they are issued identity cards) in the Capital Prague any office area of Prague 1-22 or any Registry Office , theft of identity card (but not loss) can also notify the Police of the Czech Republic.
  3. Prove your identity.
  4. Fill out an application form. You can download the form here Downloadable Forms.
  5. Submit the completed form.
  6. Pay the fee.

ValidityEdit

ID cards with Machine Readable Data

  • citizens under the age of 15 years - 5 years
  • citizens over the age of 15 years - 10 years
  • citizens over the age of 70 years - 35 years

ID cards without Machine Readable Data

  • It is valid for 6 months, if not to issue identity cards with machine readable data because of a technical malfunction of the device to preserve its production or to a disaster or other emergency.
  • It is valid for 3 months, if a citizen requests it immediately after the acquisition of Czech citizenship.
  • It is valid for 1 month at the request of a citizen in the event of loss, theft, damage or destruction of identity card, after the cancellation information on the place of residence , or if the citizen needs an identity card in connection with the exercise of voting rights ; request for his extradition is administered concurrently with the application for issuing an identity card with machine readable data.

  • Immediately report the loss or theft of an identity card to any of the following:
    • any municipal authority with extended powers (ie. where they are issued identity cards) in the Capital Prague any office area of Prague 1-22 or
    • any Registry Office ,
    • theft of identity card (but not loss) can also notify the Police of the Czech Republic.
  • You must apply for new identity card within 15 working days after the loss or theft of ID.
  • The deadline for issuing an identity card with machine readable data is 30 days
