Procedure

Apply In Person :

1. To Change a Name or Surname, the applicant has to approach the Ministry of the Interior link
2. The applicant has to visit the office with the request letter.
3. An application form will be provided to the applicant by the concerned officer.
4. The applicant has to fill in all the necessary details in the application form. (Incomplete applications are not accepted)
5. Make sure that the applicant has attached all the necessary documents that are mentioned in the “Required Documents” section of this page.
6. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
7. Submit all the documents with the completed form to the concerned officer.
8. The fee for this procedure is mentioned in the “Fees” section of this page.
9. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
10. Then the application will be processed further.
11. Once the process gets completed, the applicant gets a call from the office regarding the application.
12. The administrative authority shall decide the matter within 30 days, and for more complex cases, not later than 60 days.

Required Documents

• a birth or marriage, certificate of the applicant's birth certificate or minor child
• in the case of a divorced person, a final judgment of divorce
• in the case of persons widowed, death certificate
• proof of permanent residence in the Czech Republic (such as ID card or proof of residence permit)
• proof of citizenship (ie. an identity card, passport, certificate, or a certificate of Czech citizenship, certificate of *legal capacity for marriage, if it includes data on citizenship of the Czech Republic provided)
• proof of identity (identity card, passport, certificate of residence permit in the Czech Republic, license applicants for *international protection, certificate of residence permit of a recognized refugee)
• consent of individuals older than 15 years, in the case of a change of name or names or surnames
• in the case of a minor, the written consent of the other parent or a court decision to replace this agreement

Eligibility

A change of name or surname may be file by the following:

• Czech citizen or legal guardians of a minor citizen of the Czech Republic
• natural person who is not a citizen of the Czech Republic, if granted permanent residence in the Czech Republic

Fees

The fee is 100 CZK for this cases:

• when permission to change the surname defamatory, eccentric, funny, corrupted or foreign language
• when permission to change the surname on the previous surname
• when permission to change the surname of the child entrusted to foster care at birth guardian or foster parent to a common surname, legal guardians or foster parents or the surname agreed for their own children when guardians or foster parents, grandparents or siblings, parents of minors

Note:
For other cases, the applicant will pay 1000 CZK.

Explain the time until which the certificate/document is valid.

Instructions

• Changing surnames will be allowed especially when the case of surnames abusive or ridiculous, or if for that reason.
• The name change shall not be asking if an individual male to change the name of a female, or vice versa, asking if a name change in the name garbled, zdrobn?l, domesticated, or name that has a common living sibling parents. If doubts arise about the correct spellings for names, the applicant shall submit a document issued by an expert.
• Changing the name or surname shall not be authorized if the change was in conflict with the needs and interests of the minor.
• The fee is payable in cash or money order prior to the operation, ie. before deciding on a permit change of the name or surname. For a change of surname of spouses, parents or parents and their minor children in common surname is collected only one fee. For more change the surname of minor children of the same parents have collected only one fee.
• Exempt from the fee is to change the name, which occurs upon notification of a divorced spouse to accept once again my previous surname or that ceases to use the common surname beside the previous surname, if it was aware of this fact any registry office within six months after the judgment comes into force about divorce.
• Exempt from the fee is to change the name and surname of the person in the event of a change of gender.

Required Information

The written request must have the following information:

• the name or names, surname, maiden name, date and place of birth or date and place of marriage applicants
• personal identification number
• place of permanent residence, or a minor child
• name, or names, or surnames, which the applicant has chosen
• the name or names, surname, maiden name, date and place of birth spouse or minor children, was related to the change of *name or surname to these persons
• data on citizenship applicants, or minor child
• reasoning
