Procedure

1. Write the application as a letter.
2. Go to any of the following and submit the letter:
   1. registration office at the place of residence
   2. at the municipal office,
   3. in Prague and territoriality divided statutory cities offices boroughs or urban districts, where so provided by the statutes of these cities, or abroad, the embassy of Czech Republic
3. Present the required document. (Refer to Required Documents section for information.)
4. Pay the fee.

Required Documents

• disclose this fact in writing. Communication in paper form must contain a certified signature of the citizen; It does not apply when a citizen signs a communication prior to the registration office or employee of the embassy. If the communication form of a data message citizen after filling it signs a recognized electronic signature or deliver via data boxes
• prove the identity of an identity card or passport.

Eligibility

• Czech citizens older than 15 years or the authorized agent on the basis of a special power of attorney with notarized signatures
• legal representative for the citizen younger than 15 years or a natural person to whom the child was entrusted to the care of a court decision
• legal representative for citizen whose legal capacity was limited, so he is unable to report the completion of permanent residence
• household member whose authorization to represent citizen has been approved by the court, or his authorized agent on the basis of a special power of attorney with notarized signatures.

Fees

For notification of termination of permanent residence in the Czech Republic pays citizen at AROs administrative fee of 100, - CZK; for submission to the embassy of the Czech Republic abroad, then an administrative fee of 300, - CZK.

Explain the time until which the certificate/document is valid.

Processing Time

Registration Office writes data on termination of permanent residence without undue delay, but not later than 3 working days from the date when the creation or modification of information associated with the registered figure hear.

Instructions

• Communication is in free form.
• Termination of permanent residence in the Czech Republic expires identity card. The municipal authority with extended powers, which issued his identity card to the registry office or place of permanent residence is obligated to present an identity card within 15 working days from the notification of this fact.
