ProcedureEdit

  1. There are two ways you could apply for assignment of a birth number. You could fill out a prescribed form or write a letter with all the required information.
  2. Attached the required documents with the completed form or letter.
  3. Submit it personally or send by regular mail to

  4. Pay the fee. (Please refer to Fees section for information about payment.)

If such document is written in a language other than the Czech language then its official translation into the Czech language is required.

InstructionsEdit

  • Birth numbers assigned to individuals born before 1 January 1954 have the same structure, however are composed of nine digit numbers, having only three digits at the end, and they do not satisfy the condition of being divided by eleven.
  • An application for assignment of a birth number may be submitted by a foreign national who applies for the assignment of a birth number to comply with conditions stipulated by special legal regulations (for example Act No 48/1997 Coll. on Public Health Insurance and on the Amendment and Supplement of Some Related Acts, as amended; or Act No. 582/1991 Coll. on Organisation and Implementation of Social Security, as amended).
  • An application for assignment of a birth number must be written in Czech or furnished with an official translation.
  • It must include the officially verified signature of the applicant, but this is not, however, required in the case that the applicant signs the application in the presence of an official of the Ministry of the Interior.
  • An application for assignment of a birth number may be submitted on a prescribed form or in the form of a written letter. The form Application for Assignment of a Birth Number can be obtained from all offices where such applications can be submitted.
  • Payment is made either by a duty stamp or by transfer to the relevant bank account. The administrative fee is payable on the day of application delivery or later, however, always before the birth number is assigned.

  • first name or names, surname or their respective changes
  • maiden name
  • day, month and year of birth
  • place and district of birth; if an individual was born abroad then the place and country of birth
  • gender
  • nationality



Need for the DocumentEdit

A birth number is a ten digit number which can be divided by eleven without any remainder. The first two digits represent the year of birth; the second two digits express the month of birth - in the case of females 50 is added; and the third two digits represent the date of birth. The last four digits distinguish between people born on the same calendar day.
