Croatia - Apply for an Identity Card
ProcedureEdit
- To apply for an identity card, obtain and fill up Form 2.
- The form must be signed by the applicant confirming the accuracy entered in the form and submitted along with all other required documents.
- The official receiving the request for the issuance of identification cards shall establish the identity of the person making the request.
Note:
- In case the name of the applicant consists of several words, the name that will be printed on the card will be the name declared before the registrar and designated to be use for legal purposes.
- If the applicant is a minor older than 16 and does not have a document that can verify his/her identity, they need to bring along a parent or guardian who will verify the identity of the applicant.
- Application for the issuance of identity card for persons under the age of 16 years must be filed by a parent or guardian. If parents no longer live together, the request shall be filed by the parent with whom the individual lives based on the decision of the competent authority. Minor over the age of 12 years must be personally present when applying for the ID card in order to provide fingerprints and signatures.
- In case there are any changes on the personal information of the holder like name or surname, personal identification number, sex, date and possible correction of birth, change of residence or deregistration of permanent ex officio. The holder must apply for a new ID card within 15 days from the date of occurrence of any said circumstances.
Loss of Identity Card
- Immediately report the loss of the identity card to the nearest police station. If the lost of the identity card occured abroad, the holder is required to report the loss to the nearest Croatian diplomatic mission or consular office abroad.
- The police will need to issue a decision to invalidate the card, in this case the holder must pay an administrative fee of EUR 20.00 + 50.00, and within 3 days from the receipt of the decision, have it published in the "Official Gazette" bearing all expenses related to this procedure.
- A new card will be issued after publication at the "Official Gazette"
- Any citizen who have found an ID card is obliged to report the said incident.

- Proof of payment of the application fee.
For those applying due to the expiration of the old identity card or can no longer serve its purpose:
- Old identity card
- One colored photograph sized 3.5 x 4.5 cm
- Proof of payment of the application fee.
For those applying for a new card because of changes in their name:
- Extract from the register of births, with a note on the new name or new last name that the person will use in legal transactions, or a marriage certificate.

- Payment for the application fee shall be made to the state budget IBAN code: HR1210010051863000160 with an indication of the identity card by model HR65 7005-442-VAT, according to OIB person who will make the ID card.
- The payment for the issuance of identification cards can be made ??online or by check. Payment slip can be obtained at the police department / station. All citizens of Croatia who resides in the country are required by law to obtain an identity card. Any person who do not possess an identity card and whose aged is older than 16 years old shall be fined for the offense.
