China - Obtain a Yellow Fever Permit

Visit a health care provider (such as a doctor, physician assistant, nurse, nurse practitioner, or pharmacist) to get any vaccines, medicines, and advice you need before your trip. Going at least 46 weeks before you travel is best, so that any vaccines you need have time to take effect and you have plenty of time to get vaccines that require more than one dose.
If you are visiting several countries or countries with a variety of health risks, or if you have a pre-existing health condition, you may need to see a clinician who specializes in travel medicine.
If your travel plans include traveling to or from a South American or African country that is infected with yellow fever or is located in areas where yellow fever transmission has occurred (ENDEMIC AREAS), then the easiest and safest thing to do is to get a yellow fever vaccination and a signed certificate. Be aware, the age requirements for vaccination can vary from country to country.
After immunization an International Certificate of Vaccination is issued and is valid 10 days after vaccination to meet entry and exit requirements for all countries.

- A valid original passport or travel document and photocopies of the photo-page, expiry-date-page and info.-update-page of your passport or travel document
- Four recent 2-inch photos (full face without hat)
- If applying for working, applicants should also bring a photocopy of your companys business license or registration certificate
- If applying for studying, applicants should also bring a photocopy of your students ID or admission notice
- For those coming to China only as accompanying family members, they should provide a photocopy of family members business license

ValidityEdit
The yellow fever vaccine is valid for 10 year since the day of inoculation. If the second inoculation follows in no more 10 years, the period of validity of the vaccine is 10 year since re-inoculation. People from the yellow fever prevalent area should note the validity of their vaccination before departure.

Need for the DocumentEdit
Yellow fever, also yellow plague or yellow jack, is an acute virus disease. The infected person would have the symptoms of fever, chillness, loss of appetite, nausea, pain in muscle (especially in back) and headache. These symptoms usually improve in 5 days, while some people have a fever again after 1-2 days, following abdominal pain and probably leading to kidney failure.

All travelers should visit either their personal physician or a travel health clinic 4-8 weeks before departure.
If you arrive from a country without yellow fever, there is no requirement for a yellow fever vaccination.
Travelers who have a medical reason not to receive the yellow fever vaccine should obtain a medical waiver.
Other uses of the Document/CertificateEdit
The WHO recommends that all travellers to countries with yellow fever be vaccinated.
Countries with yellow fever can request a valid vaccination certificate from travelers, and countries without yellow fever can request a certificate from travelers entering from a country with yellow fever.

If one holds an invalid vaccination certificate upon arrival, the Inspection & Quarantine Authority can have a check-up detention till the sixth day, counting from the day he/she left the contaminated place. Or directly ask him/her to inoculate immediately in local travel healthcare center and then check after six days.
