As a first step, get the FIR from the applicable police station for the lost or stolen permit. If you have the damaged piece of permit, then you can submit the same as proof.

To apply for duplicate Vehicle Permit in Chhattisgarh the applicant has to approach the RTO office to which his area belongs to.

Make sure you have completed application form for duplicate Vehicle Permit. Application forms can be obtained from the appropriate RTO offices that can be completed and presented with appropriate fees.
Please make sure you have all the documents that are listed down the “Required Document” section of this page.
Present the completed application forms in one of the counter and pay the appropriate fee where the officials would start processing your application.
After checking the vehicle for its correctness and other appropriate requirements the application will be processed. Important verifications for the permit includes Vehicle condition etc., will be carried by authorities.

Apply Online

Use the following link : Online and select “Online permit” option under permit menu.
In the next screen select other online service option and click “permit online” option.
Select Chhattisgarh in the drop down menu and click “proceed”
Please follow the screen and fill in prompted details to process your online procedure
Upload required documents and take printout of acknowledgement.
On the appointment date, go to RTO office before 11:30 am and submit the documents to officer at the office. He will mark the amount need to be paid at the respective counter.
Pay the fees and submit the document at the counter.
Authorities will process your application further.

To Apply for a Duplicate Vehicle Permit in Chhattisgarh, the applicant shall visit the following webpage: Link.
Now click on the "Permit" option and then proceed.
A new page will open up, where the applicant needs to log in using their login credentials.
After login, the applicant shall choose an appropriate option for a permit and proceed further.
On a newly opened page, the applicant shall fill up the application form with all the necessary details and complete the given application form.
Now the applicant shall upload scanned copies of all the " Required Documents" for further processing.
Once the documents are uploaded, provide the mobile number and enter the received OTP.
Now click on "Submit" and complete the process.
On successful verification, the permit will be issued.
Obtain the permit from the concerned office.
