This procedure explains various ways to Replace a Lost or Stolen or Damaged Passport in Caloocan.
Apply OnlineEdit
- Here are the procedures on how you can replace a lost, stolen or damaged Philippine Passport online. First, you will need to book an appointment using DFA’s Passport Appointment System. Click this - LINK.
- To begin, click the “Schedule an Appointment” button.
- Read the Terms and Conditions page. After reading, proceed with the booking process by clicking the “START INDIVIDUAL APPOINTMENT” button. NOTE: Option to book an appointment for a group is also available.
- The appointment page will open. Here you will be asked to indicate where you would like to process your application. Make use of the drop down menus to select your region and country. For applicants in the Philippines, you don’t need to change anything as the default setting is already set locally (DFA sites within the Philippines).
- Next, indicate the specific DFA site where you would like to process your application. Use the drop down menu and scroll through different DFA offices.
- After selecting a DFA site, click the “NEXT” button.
- Next, you will have to set a date and time for your appointment. If you notice, the calendar is set to show the current month and year. In case you would like to apply for a different month or a different year just click on the month and the year text.
- Once you have set a date and time for your appointment, tick the captcha box and then click the “NEXT” button
- Complete the Personal Information Form.
- After you have finished filling out the Personal Information Form click the “NEXT” button. Next, you will need to fill out the Family Information Form. Again, once you're done, click the “NEXT” button.
- On the next page, you will be asked what type of application you are applying for. Since you are applying to replace your lost or stolen passport, select the application type as “Lost” from the drop down menu.
- Complete the rest of the form. You will need to provide your old passport number, the date it was issued and the issuing authority. You will also need to state the basis of your Philippine citizenship and designate an emergency contact person. Click "Next" after.
- Now, enter your contact information. Click "Next" after.
- You will be taken to the Information Review page. Go over your application once more before finally submitting it for processing. Make sure that you double check everything (specially your name) because the DFA will exactly use the data that you have encoded when they process and print your passport.
- When you finish reviewing your application, tick the captcha and consent boxes, and then, click the “CONFIRM” button to finally submit your application to the DFA.
- Now, you are done with the first part of the application process which is setting up an appointment with the DFA. DFA will email you a copy of your completed form. Print and bring that form with you on the day of your appointment along with all the other required documents.

Apply In PersonEdit
- After you have set up your appointment with DFA online, to complete the replacement process you need to go to your selected DFA office or branch and submit your request in person.
- On the day of your appointment, print and bring with you the form which DFA emailed to you. If possible arrive at least 30mins before your scheduled appointment.
- Get a queue number at the Information Counter and sit on the waiting section of the Processing Area. Wait for your number to be called.
- Once called, present all documents for review by the officer.
- Pay the applicable passport processing fees plus penalty.
- After payment, proceed to the Encoding Section for data capturing. This is the last part of the process. You can personally pick up your new passport once it’s ready or, you can also have it delivered to your home (optional).
- When you collect your new passport don't forget to bring the Official Receipt.

- Original and photocopy of Philippine Statistics Authority (PSA) Authenticated Birth Certificate on Security Paper. Note: If photocopy of the applicant's last issued passport is available, the Birth Certificate is no longer needed.

- Lost EXPIRED ePassport (it is like you are applying for a NEW passport):

- Penalty for Lost and Mutilated ePassport – Php 350.00
Notes:
- For passports applied overseas, please see the consular fees stated in the Philippine Embassy or Consulate websites. The cost can be around $60.00.
- Delivery fee not yet included and it may vary depending on the location.
- Additional convenience fee for online epayments may be charged per applicant.

Processing TimeEdit
It will take about twelve (12) working days for your application to be processed. The expedited process takes 6 (within Metro Manila) to 7 working days (outside Metro Manila).
Note: Additional 15 calendar days clearing period for Lost Valid Passports.
