This procedure explains various ways to Replace Lost or Stolen Government Service Insurance System (GSIS) eCard in Caloocan.

Apply In PersonEdit

Lost GSIS eCardEdit

  1. Here's how you can replace your lost GSIS eCard. First thing that you should do is call or send an email to your Servicing Branch (the one who issued your card) and inform them that your eCard Plus is missing. They will then block your account to prevent others from using it.
  2. Next, you need to go to a notary public and execute an affidavit of loss. The notary public will ask you a few questions like your name, address and details about your lost card (when was the card lost and the issuing bank). You will also be asked to present a valid ID to confirm your identity and pay the notarial fee.
  3. Now that you have an affidavit of lost you can go to your servicing branch and fill out an eCard replacement form.
  4. Submit the completed eCard replacement form to the bank personnel together with the affidavit of loss.
  5. Your servicing branch will text or email you to notify you when can get your new eCard Plus.
  6. Collect your new card from the servicing branch where you applied.

Forgotten/Lost PINEdit

  1. You can also ask for your eCard to be replaced in case you forgot or lost your PIN. You need to go to your servicing branch to file your request.
  2. Complete and submit an eCard replacement form.
  3. Your servicing branch will text or email you to notify you when can get your new eCard Plus.
  4. Collect your new card from the servicing branch where you applied.

Damaged GSIS eCardEdit

  1. Here's how you can replace your damaged GSIS eCard. You need to go to your servicing branch to file your request.
  2. Present your damaged eCard.
  3. The branch personnel will ask you to fill out an eCard replacement form.
  4. Submit the completed form and pay the corresponding replacement fee.
  5. Your servicing branch will text or email you to notify you when can get your new eCard Plus.
  6. Collect your new card from the servicing branch where you applied.

Apply OnlineEdit

Replace Expired GSIS eCardEdit

  1. This method is only available for eCard Plus holders based abroad. To start the replacement process, send an email request for Skype Video Call Conference to [email protected].
  2. Wait for confirmation of schedule from the pensionglobal mailbox.
  3. On the date of your appointment please have your ecard Plus and/or scanned/photo copy of ecard Plus/UMID and/or 2 Valid IDs or Passport ready. Also, make sure that your computer is positioned in a well-lit area.
  4. Log in to your Skype username/account at least 15 minutes before your scheduled time. Wait for a call from GSIS using these following Skype username/accounts: gsis.global1 / gsis.global2 / gsis.global3 / gsis.global4 / gsis.global5 / gsis.global6.
  5. During the video call, an interview will be conducted by GSIS Skype Interviewers for the processing of eCard Plus replacement.

  • A GSIS eCard Plus is an all-in-one card that allows you to transact with GSIS. It looks like a regular ATM Card and can be used as one. It can also be used as a debit card. You may present your eCard Plus as a hospitalization discount card, medicine discount card, and tuition discount card. In the event that you lost your eCard, you need to immediately apply for a replacement so that the banks and GSIS can cancel your old one and issue you a new one.
