Where the death of a Botswana citizen occurs inside a medical facility

  1. In the case where the death of an individual has occurred in any government approved medical facility, the doctor or a medical practitioner will confirm the cause of death and register the death in triplicate and issue a designated family member/friend or next of kin, two (2) copies of the death notification certificate/card outlining the particulars in relation to the death.   2. Once you have the death notification card/certificate, present it attached to the supporting documentation as mentioned under the “Required Documents” section of this page, to the Office of the District Commissioner registrar’s office within the area where the burial or cremation ceremony will take place. (Refer to the “Office Location & Contact” section of this page for the District Commissioners contact details).
  3. Upon submission, the application is checked for completeness and correctness by the registrar, if everything is in order the burial permit is issued to the applicant, immediately for free.


Where the death of a Botswana citizen occurs outside a medical facility

  1. In the case where the death of an individual has death occurred outside a medical facility, the death should be reported to the nearest police station.Botswana Police Service Contact Details
  2. The police officer in charge will conduct an interview with the person reporting the death to establish the cause of death and relationship to the diseased.
  3. The police officer will the issue the death notification certificate/card outlining the particulars in relation to the death, to a designated family member/friend or next of kin against their identity card.
  4. Once you have the death notification card/certificate, present it attached to the supporting documentation as mentioned under the “Required Documents” section of this page, to the Office of the District Commissioner registrar’s office, within the area where the burial or cremation ceremony will take place.
  5. Upon submission, the application is checked for completeness and correctness by theregistrar, if everything is in order the burial permit is issued to the applicant, immediately for free.


Foreign nationals wishing to bury or cremate thedeceased in Botswana.

  1. In the case where the death of an individual has death occurred in any government approved medical facility, the doctor or a medical practitioner will confirm the cause of death and register the death and issue a designated family member/friend or next of kin, the death notification certificate/card outlining the particulars in relation to the death.
  2. In the case where the death of an individual has occurred outside a medical facility, it should be reported to the nearest police station immediately, the notifier is then issued with a death notification certificate.
  3. Applicants should then contact the diplomatic mission, or consulate of the country of the deceased in Botswana and request a Note Verbale authorizing burial in Botswana or repatriation from Botswana.
  4. Submit the Note Verbale and accompanying documents (A copy of the Death Notification certificate, a copy of passport or Identity card of applicant, residence permit, and letter from spouse or authorized immediate family member consenting to the burial)to the Office of the Department of Protocol and Consular Services, housed at the Ministry of International Affairs and Cooperationoffice, for processing and consideration which is within 2 working days.
  5. If approval, the applicant is issued with an approval letter from the ministry, the applicant shouldthen head to the District Commissioner, registrar’s office, within the area where the burial or cremation ceremony will take place or where the death occurred to get a burial authorization.
  6. Submit the ministry approval letter attach to the supporting documentation, the application is checked for completeness and correctness byregistrar’s officeand if everything is in order, the application is registered for processing and consideration by the District Commissioner/authorized official, which is within a day.
  7. If approved, the burial permit is drawn up and made available for issue to the applicant against their identity document and theDeath Notification certificate.

  • A copy of the National Identity Card (Omang)/Botswana Passport of the deceased or any other document certifying the deceased identity (birth certificate).
  • A copy of the Death Notification certificate or death certificate.
  • Note Verbale from country's representative Diplomatic Mission or Consulate(in case of a deceased foreigner)
  • Letter of consent from spouse or authorized immediate family member(in case of a deceased foreigner)
  • Copy of passport of the deceased.(In case of a deceased foreigner)
