How to Apply
Apply In-Person

  1. The Application for conveyance or release of a corpse is only processed Ministry of Health and Wellness, Department of Health service Headquarter office during working hours {see address details below}.
  2. Applicants should visit the ministry office and Pick up the control ticket and wait for their turn at the waiting section, submit the supporting documents {as listed under required document section below} to the department responsible ,attending officer for verification and processing.
  3. The officer is satisfied that all the documents are in order he/she will issue the applicant with a conveyance/release authorization permit immediately.

Required DocumentsEdit

  • Three certified copies of death certificate.
  • Three copies of non-infectious letter from the hospital.
  • Three certified copies of passport of deceased.
  • Three certified copies of passport of the accompanying person with the same surname (in case of different surnames, affidavit from police to be availed).
  • If person died at home, three copies of police affidavit needed.
  • Death certificate should always show cause of death, if not, affidavit from police to be availed to confirm there is no foul play.

  • The circumstances under which it is desired to remove the body into Botswana
  • The proposed mode of conveyance of the body
  • The precautions which will be taken to prevent any danger to health or any cause of offence arising,
  • The proposed mode and place of disposal of the body in Botswana
  • Date on which the body will be buried/cremate
  • Declaration by the applicant
