Procedure

How to Apply

1. The Application for immunization clearance letter is processed Ministry of Health and Wellness, Child Health Division {see address details below}.
2. The application file can either be submit in person or via mail.
3. If the application is to submit in person, the applicant should visit the Ministry of Health and Wellness, Child Health Division office during working hours and submit the original copies of the supporting documents {listed below under required document section} to the attending officer to be verified and processing.
4. If the file is submitted via email, certified copies of the supporting documents should be forwarded.
5. Upon receipt of the application file, the Child Health Division officer will process the application file and issue you with an Immunization Clearance Letter immediately, if the application is made in person or send it within one working day if it is by email.

Required Documents

• Certified copy of applicable identity document {ID card, passport, Residence permit, Driver’s license}
• Certified copy of Child Welfare Card (CWC) or under five card or an affidavit in the absence of the card.
