Procedure

Apply In-Person:

1. Applicant shall approach the Public Facilitation Centre, Deputy Commissioner office .
2. Link for contact. link1 and Municipal office contact link2 Please select your ULB from the drop down menu under Online citizen service centre and submit. User will be directed to respective municipal website. Click the contact menu or other appropriate option to know the address.
   
3. Obtain 'Birth Certificate Update/correction Form' from the respective counter or write in a paper to apply.
4. Please complete the application form and attach the required documents and submit it to the receiving authority
5. The accepted application and documents will be verified by the authority and a acknowledgement will be provided for applying.
6. Collect the acknowledgment slip after submitting the form and keep it safe for future reference.
7. This application will be processed further by the respective department.
8. Once the process is over, applicant gets notification to collect the certificate.
9. Use the acknowledgment slip to obtain certificate from the office as per notified period.
   

Note: This changes will be published in a local newspaper and with the respective state official Gazette publication.

Apply Online:

1. Visit the following portal: portal.
2. Fill in all the personal details and the shipping address.
3. Now, the applicant will be redirected to the payment page.
4. On successful payment, the applicant will receive an application number as acknowledgement.
5. Upon the successful approval of the concerned authorities, the applicant will be permitted for the certificate.
6. The certificate will be sent to the provided shipping address within a stipulated period of time.

Apply through Centres:

1. To apply through centres, the applicant has to visit the respective “CSC” centre. Please tap on the following link to locate the centre according to the respective district: link.
2. Make sure to have the documents that are listed under the “Required Documents” section of this page.
3. An application form can be obtained through the kiosk operator upon visiting the centre.
4. The applicant has to fill in all the necessary information in the application form.
5. Submit the application form with ”Required Documents” to the office.
6. It is advised to take the documents in original to the kiosk for scanning purposes.
7. The operator will process the application online.
8. On successful submission, the applicant will receive an application number as acknowledgement.
9. If approved, the updated certificate will be hand delivered or sent through post to the address mentioned while applying in a stipulated period of time.
