A Death Certificate is a valid and official document providing information about a deceased person. The following information below will discuss the process of obtaining it.

  • Father's Full Name
  • Mother's Full Name
  • Signature of Applicant

Procedure

  1. Complete the form.
  2. Submit the form to the Deaths registry.
  3. A search for the certificate will be made.
  4. If found, payment is made and certified copy is issued.
