Procedure

Apply In-Person

1. To Apply for a Med-Card the psychiatric patient or applicant on behalf of the psychiatric patient shall submit an application request at the Health Social Services (HSS) Division at the Sandilands Rehabilitation Centre (SRC).
2. The Social Service Representative (SSR) will call for an interview with the psychiatric patient or applicant to assess the patient’s eligibility.
3. The Med-Card Summary form will be completed by the SSR and will submit a recommendation about the applicant’s eligibility.
4. The Social Service Representative (SSR) will inform the applicant about their eligibility on the same day of the application
5. Med-Cards for the patients are issued to the patients or the applicants on the same day of application.

For Citizens:

• Valid Passport of the patient and the applicant.
• Valid N.I.B. Card of the patient and the applicant.
• Copies of prescriptions.
  

For  Permanent Residents:

• Proof of Citizenship/Permanent Residency of the patient and the applicant.
• Copies of prescriptions.

With the exception of Grand Bahama, this service is not offered directly from a Family Island sub-office.

Eligibility

The eligibility criteria are as follows:

• Any Psychiatric patient earning minimum wage or lower.
• Any Bahamian Citizen or Permanent Resident.
• All Psychiatric patients must be over 18 years to 64 years.
  

Fees

There are no fees associated with this service.

Validity

The Med-Cards are provided to psychiatric patients for periods ranging from one day to six months.

Processing Time

• On the same day of the application, a Social Service Representative (SSR) contacts the applicant and informs about the decision.
• Med-Cards for PMH patients are issued on the same day as the application is submitted.
• Reassessments for Med-Cards are conducted at six-month intervals.
