Apply In-Person

  1. The Applicant must visit the Senior Citizens Division and ask the Social Services Representative for assistance (SSR). Visit the following link for Contact Details:Contact Link.
  2. SSR then completes the intake procedure and the form.
  3. Following that, a needs assessment and a house investigation are done.
  4. For each item requested, the applicant must submit three price quotations.
  5. On the basis of the findings, the SSR provides a recommendation.
  6. The applicant is notified of the decision of whether or not he or she is eligible for the service.
