This procedure explains various ways to Replace Lost or Stolen or Damaged Student Driver Permit in Bacolod.

Apply In-PersonEdit

  1. Here's how you can replace your lost, stolen or damaged student driver's permit at Land Transportation Office (LTO). You may apply for a duplicate permit at any LTO licensing office near you. But, check first if the LTO office of your choice process driver's license applications. Next, check if you need to book an appointment online with LTO ahead of your visit. Note: LTO just launched a new website portal called the Land Transportation Management System (LTMS) which you can use to set up an appointment and transact with LTO. But not all LTO offices in the country is connected to the system. Some offices may still accept walk in applicants. It is very important that you confirm first before you go to a LTO office.
  2. For those who have lost their permit, you will need to prepare an Affidavit of Loss. To help you draft one, you may go to a notary public.
  3. At the LTO office, ask where can you get an Application for Driver’s License (ADL) form. To save you some time, you may also check the link provided under 'Documents to Use' section which you can click to download the Application for Driver’s License (ADL) form. Once you have completed the form present it to the LTO officer together with your other requirements and get a queue ticket. Wait for your number to be called.
  4. Once called, submit all the required documents.
  5. Proceed to the Cashier to pay the application fee.
  6. Go to the encoding / biometric window and complete the form for encoding of your details into the LTO-IT system.
  7. You will be informed by the LTO officer when can you come back to collect your OR (Official receipt) and your student permit.
  8. When you pick up your permit, you will be asked to sign in the log sheet before the permit is released to you.

ValidityEdit

A student permit is only valid for 1 year from the date it was issued and it may not be revised, renewed or modified.
