This procedure explains various ways to Delayed Death Registration in Bacolod.

Apply In PersonEdit

For deaths that occurred in the Philippines

  1. Here's how you can register a death that occurred in the Bacolod beyond the 30-day period (delayed registration). First, you will need to check that there is no existing record of the said death. You can do this by inquiring at your local civil registry (where the death occurred).
  2. After inquiring at your local civil registry, visit any Philippine Statistics Authority office and ask them to confirm that there is no existing records in their database related to the death. You will also need to secure a Negative Results Certification (NRC) from PSA. This is one of the requirements you will need to submit when you begin the late registration process.
  3. You will also need to secure an affidavit executed by the hospital/clinic, the nearest relative or the person who is acting as administrator or attendant at death, if it occurred in the said institution or under the care of the attendant. It must be mentioned in the affidavit the name of the deceased, the facts of his death, the date and place of burial or cremation, and the circumstances why the death was not registered within thirty (30) days after death.
  4. Compile all the documents you will need to submit to your local civil registry. See list provided under 'Required Documents' section.
  5. Once you have all the needed documents head to your local civil registry to file your application for late registration of death.
  6. Pay the late registration fee.
  7. Your application will be evaluated by the civil registrar. A notice will be posted on the bulletin boards of the city or municipality for a period of not less than ten (10) days to inform the public on your pending application. You will be advised by the civil registrar to return after the said period.
  8. If no one opposes the application it will be approved and signed. And a copy of the death certificate will be released to the registrant.


For deaths that occurred abroad
Report death of a Filipino who died abroad BEFORE 2020:

  1. Here are the procedures you will need to follow to register a death of a Filipino who died abroad before 2020 (different procedure for Filipinos who died abroad in the year 2020 onwards). For this, application will be processed by the Department of Foreign Affairs, Office of Consular Affairs (if the registrant is in the Philippines) or the Philippine Consul's Office in the country or place where the Filipino died (for registrants who are currently based abroad).
  2. First, you will need to prepare all the needed requirements. Please note that documentary requirements may vary depending on where the death occurred and the Consular Office processing the registration. Provided under 'Required Documents' section are some of the documents you will be required to submit. You may also check your local Consular Office's website in case the office requires additional documents to be submitted.
  3. Submit your application and all the required documents to DFA's Office of Consular Affairs or the Philippine Consul's Office.
  4. Pay the consular fee.
  5. Applications are normally processed in 2 to 4 months. Some consular offices issues a tracking number to their applicants to make it easier for them to monitor the progress of their application. In case you don't have a tracking number it might be good for you to take note of the contact details of the consular office processing your application. This way you can give them a call to follow up.


Report death of a Filipino who died abroad IN 2020 onwards:

  1. Before you proceed with the application process please verify first if the death was already registered at the Foreign Service Post having jurisdiction over where the death took place. You can do this by sending an email to [email protected].
  2. Once you have confirmed that the death hasn't been recorded yet you may now file an application to register the death (follow procedures above).

Note: The Department of Foreign Affairs, Office of Consular Affairs are processing applications on APPOINTMENT BASIS, you will need to email [email protected] to set an appointment.

Required DocumentsEdit

The death occurred in Philippines:

  1. Correctly and completely accomplished Certificate of Death (4 copies). There's a box provided in the Certificate of Death that must bear the approval for registration by the health officer. When you submit it make sure that it is signed by the health officer.
  2. Affidavit executed by the hospital/clinic, the nearest relative or the person who is acting as administrator or attendant at death, if it occurred in the said institution or under the care of the attendant. It must be mentioned in the affidavit the name of the deceased, the facts of his death, the date and place of burial or cremation, and the circumstances why the death was not registered within thirty (30) days after death. It must be accomplished and notarized or subscribed by authorized Officer.
  3. Authenticated copy of the certificate of burial, cremation, or of other means of corpse disposal.
  4. Certificate of No Record issued by the Civil Registrar.
  5. Negative Certification issued by the Philippine Statistics Authority.

The death occurred abroad:

  1. Report of Death form
  2. Death Certificate (original copy should be submitted to the DFA)
  3. For death certificates not in English language, you will need to submit an official English translation
  4. Certificate of Sealing of Casket
  5. Autopsy/Embalming Report
  6. Photocopies of Passport of the deceased (data pages only)

Notes:

  • You will need to present original documents and submit five (5) photocopies of each of the documents listed above.
  • The DFA officer or consular officer may request you to submit additional documents upon assessment of the application, depending on the circumstances of death.
  • To those submitting their application through the Philippine Consul's Office or Embassy abroad, you may also check their website for any additional documents they will need to assess your application.

EligibilityEdit

  • The death was not recorded with the corresponding Local Civil Registry Office (LRCO) if the death occurred in the Philippines and the DFA Office of Consular Affairs if the deceased died abroad. And, in cases where the death was already recorded by the LCRO but the records have been destroyed or cannot be located despite diligent search and there is no existing copy with the Philippine Statistics Authority (PSA). The death may still be registered.
  • Relatives or representative of the deceased may file the application for delayed registration.

FeesEdit

  • Local Civil Registry Office: Late registration fee varies per city or municipality. It usually ranges from Php50-Php100.
  • Office of Consular Affairs (DFA): Consular Fee is USD 25.00 to be paid in Philippine peso.
  • Philippine Embassies, Consulates and Missions Abroad: Varies per office (fees are usually available on their website).
