This procedure explains various ways to Correct Entries in the Voter Registration Record in Bacolod.

For overseas Filipino votersEdit

  1. Prepare the requirements:
    • Your valid Philippine passport, or a certification from the Department of Foreign Affairs (DFA) in lieu of a passport
    • For seafarers, a photocopy of the Seaman’s Book
    • For dual citizens or those who reacquired or retained Filipino citizenship via Republic Act 9225, the original or certified true copy of the Order of Approval, or the Identification Certificate issued by the foreign posts or the Bureau of Immigration.
    • Application for Correction of Wrong Entries/Misspelled Names/Change of Name and Other Entries in the Voter's ID/National Registry of Overseas Voters (NROV) Application
  2. Apply for Correct Entries in the Voter Registration Record
    Visit a suitable office in the Field Registration Centers Office Locations or the List of Consular Jurisdictions Refer List to accomplish the above OVF No 1D form or OAVF No 1E form. Present the required documents and follow all of the officer’s guiding to correct entries in the Voter Registration Record.

For general votersEdit

  1. Prepare the requirements:
    One of the valid identification documents:
    • Employee’s identification card (ID), with the signature of the employer or authorized representative;
    • Postal ID;
    • PWD Discount ID;
    • Student’s ID or library card, signed by the school authority
    • Senior Citizen’s ID;
    • Driver’s license;
    • NBI clearance;
    • Passport;
    • SSS/GSIS ID;
    • Integrated bar of the Philippine (IBP) ID;
    • License issued by the Professional Regulatory Commission (PRC);
    • Certificate of Confirmation issued by the National Commission on Indigenous Peoples (NCIP) in case of members of ICCs or IPs; or
    • Any other valid ID
      • In the absence of any of the above-mentioned identification documents, you may be identified under oath by any registered voter of the precinct where you intend to be registered, or by any of your relatives within the fourth civil degree of consanguinity or affinity. No registered voter or relative shall be allowed to identify more than three applicants.
      • Community Tax Certificate (cedula) and PNP clearance shall not be honoured as valid identification documents for this.
    • The Application for Change / Correction of Entries / Inclusion of Record in the Book of Voters and Reinstatement of Name in the List of Voters Application

  2. Apply for Correct Entries in the Voter Registration Record
    1. Visit a suitable office of the Commission on Elections Office Locations and present the required documents. You should personally accomplish in three copies the CEF-1D application form. The election officer (EO) shall check the box corresponding to the appropriate type of application.
    2. The EO shall verify whether your name is found in the database. If the result is in the affirmative, the EO shall immediately report the incident to the EBAD email: [email protected] and ITD email: [email protected] for technical assessment of correct the entries in the Voter Registration Record.
    3. In the event that your name is not found in the database, despite the fact that you have a Voters Registration Machines (VRM) in the precinct book of voters, you will be directed to the VRM operator who shall encode your demographics using the entries in your Voters Registration Record (VRR) and thereafter capture your biometrics. After the Election Registration Board (Board) approval, the EO shall correct the wrong entries in VRR and consolidate/merge the data.
    4. If it is denied or not acted upon, you may file with the proper Municipal Circuit, Municipal or Metropolitan Trial Court a petition for an order directing that your name be entered in the list. You should attach to the petition, a certified list of voters used in the preceding election, together with proof that your application were denied or not acted upon by the Board and that you have served notice to the Board.

InstructionsEdit

  • The applicant shall attach the court order or an order of the Civil Registrar, or any other evidence warranted under the circumstances when filing his or her application.
  • The application form may also be obtained free-of-charge from the Election Officers office.
  • The application form can be downloaded here: Application Form for Correction of Entries. It is to be printed on a long bond paper (preferably substance 20/70 gsm), after which the applicant shall manually fill-up the same in three (3) copies.
