Procedure

This procedure explains various ways to Change of Name in Voter Registration Record in Bacolod.


You are a Bacolod citizen or resident. You were a registered voter. But your name is changed by reason of marriage or by virtue of a court order. You may personally, or through your authorized representative, file an application for change of name in voter registration record with a suitable election officer or post not later than ninety days before the start of voting period. Please follow the guide below:

For overseas Filipino voters

1. Prepare the requirements:
   
   • Your valid Philippine passport, or a certification from the Department of Foreign Affairs (DFA) in lieu of a passport with your new name inside
     
   • A photocopy of the Seaman’s Book with your new name inside (For seafarers)
     
   • For dual citizens or those who reacquired or retained Filipino citizenship via Republic Act 9225, the original or certified true copy of the Order of Approval, or the Identification Certificate with your new name inside issued by the foreign posts or the Bureau of Immigration
     
   • Application for Correction of Wrong Entries/Misspelled Names/Change of Name and Other Entries in the Voter's ID/National Registry of Overseas Voters (NROV) Application
     
     
2. Apply for change of name in voter registration record.
   

Visit a suitable office in the Field Registration Centres link or the List of Consular Jurisdictions Office Locations to accomplish the above OAVF No 1E form. Present the required documents and follow all of the officer’s guiding to change of name in voter registration record.

For general voters

1. Prepare the requirements:
   One of the valid identification documents that contains your new name:
   
   • Employee’s identification card (ID), with the signature of the employer or authorized representative;
     
   • Postal ID;
     
   • PWD Discount ID;
     
   • Student’s ID or library card, signed by the school authority
     
   • Senior Citizen’s ID;
     
   • Driver’s license;
     
   • NBI clearance;
     
   • Passport;
     
   • SSS/GSIS ID;
     
   • Integrated bar of the Philippine (IBP) ID;
     
   • License issued by the Professional Regulatory Commission (PRC);
     
   • Certificate of Confirmation issued by the National Commission on Indigenous Peoples (NCIP) in case of members of ICCs or IPs; or
     
   • Any other valid ID
     
     • NOTE: Community Tax Certificate (cedula) and PNP clearance shall not be honoured as valid identification documents for this.
       
   • The Application for Change / Correction of Entries / Inclusion of Record in the Book of Voters and Reinstatement of Name in the List of Voters Application
     
     
2. Apply for change of name in voter registration record:
   1. Visit a suitable office of the Commission on Elections Office Locations and present the required documents. You should personally accomplish in three copies the CEF-1D application form.
   2. The election officer (EO) shall check the box corresponding to the appropriate type of application. The EO shall verify whether your old name is found in the database. If the result is in the affirmative, the EO shall immediately report the incident to the EBAD [email: [email protected]] and ITD [email: [email protected]] for technical assessment of change the old name to new name.
   3. In the event that your name is not found in the database, don’t worry because you have a Voters Registration Machines (VRM) in the precinct book of voters, you will be directed to the VRM operator who shall encode your demographics using the entries in your Voters Registration Record (VRR) and thereafter capture your biometrics and then proceed to update the new name. After the Election Registration Board (Board) approval, the EO shall consolidate/merge the data.
   4. If it is denied or not acted upon, you may file with the proper Municipal Circuit, Municipal or Metropolitan Trial Court a petition for an order directing that your name be entered in the list. You should attach to the petition, a certified list of voters used in the preceding election, together with proof that your application were denied or not acted upon by the Board and that you have served notice to the Board.

Instructions

• The applicant should attach supporting documents to prove the change of name.
• The application form can be obtained free-of-charge from the Election Officer's office. It can be downloaded here: Application Form for Change of Name. It is to be printed on a long bond paper (preferably substance 20/70 gsm), after which the applicant shall manually fill-up the same in three (3) copies.

Need for the Document

This procedure is appropriate to those registered voters whose names have been changed by virtue of marriage, or by court order or order by the Civil Registrar or Consul General.

Information which might help

This application may also be availed of in cases where the marriage of a female registered voter has been annulled or declared as void by competent court. In this case, the affected voter may request to revert back to her maiden surname and for the change of signature. The applicant must attach a certified true copy of the Courts final Order or Decision, annulling the marriage or declaring the marriage a nullity.
