This procedure explains various ways to Apply to Continue Your PhilHealth Coverage (Voluntary Contribution) in Bacolod.

Apply In PersonEdit

  1. Here's how you can apply to continue your PhilHealth coverage or in other words contribute voluntarily to the Fund. You usually apply for this procedure after you have resigned from your previous job to become a full-time freelancer, OFW, or an entrepreneur. You can continue paying your contribution as a self-employed individual or migrant worker. But first, you will need to update your PhilHealth Member Data Record (MDR).
  2. To update your record, download the PhilHealth Member Registration Form (PMRF). A link is provided under 'Documents to Use' section which you can click to download the form. In case you do not have a printer at home, don't worry forms are also available at the PhilHealth office.
  3. Complete the application form. On the upper right corner of the form, for the "PURPOSE", check the "UPDATING/AMENDMENT" box. Also, when you get to the "IV. MEMBER TYPE" section (bottom part of the form) check the appropriate box that corresponds to your situation:
    • For OFWs, check boxes, "Migrant Worker", and "Land Based" or "Sea Based".
    • For self-earning individuals or freelancers, check the box, "Individual" or "Sole Proprietor".
  4. You are also required to provide your profession and monthly income on the form. The monthly income is important because your monthly contribution will be based on the amount of you will provide on the form.
  5. Once again, go over your form and make sure that all the information are correct. In case you also want to update or amend or change any data on your MDR you can do so by completing the "V.   6. After completing the form, go to any PhilHealth branch and submit your application and other supporting documents.
  7. Wait as the officer validates your information and encode it on the PhilHealth database.
  8. After updating your Member Data Record (MDR) the officer will issue a new MDR and PhilHealth ID.
  9. Now that you have registered as a voluntary member of PhilHealth paying the monthly contributions is now your responsibility. Take note of the amount of your monthly contribution and due date.

  • Completed PhilHealth Member Registration Form
  • For OFWs you may be asked by the PhilHealth officer to present any of the following:
  • Overseas Employment Certificate (OEC)
  • Employment Contract
  • Working visa/re-entry form
  • Company ID issued by your overseas employer
  • Cash remittance receipt for money sent from abroad (at least two months before)
  • Valid ID or certificate issued in the country of employment

Notes:

  1. As stated on circular the rates increases yearly. So, when you compute for your monthly contribution make sure that you double check the year and the rate that you are using.
  2. For self earning individuals, to compute for your monthly contribution use this formula, Monthly Income (the amount you wrote on the registration form) multiply by the premium rate (for 2021 it's 3.50%).
  3. Payment can be made monthly, quarterly, semi-annually or annually.
