This procedure explains various ways to Apply for Government Service Insurance System (GSIS) eCard in Bacolod.

Apply In-PersonEdit

  1. To apply for a GSIS eCard or a UMID (Unified Multi-Purpose ID) or a Temporary Card, members and pensioners will need to visit a GSIS office. You will be asked to present two valid government-issued IDs (examples: LTO license, PRC license, passport, voter’s registration ID).
  2. Complete an eCard enrollment form. You can ask a GSIS personnel for a blank form or you can also download one using this link.
  3. When filling up the form you will see that there's a section asking you about your preferred servicing bank. You may choose between Union Bank of the Bacolod (UBP) or Land Bank of the Bacolod (LANDBANK). Indicate your chosen bank by checking the box that corresponds to your choice. Note: Pensioners who chose LANDBANK as their servicing branch will no longer be allowed to transfer their UMID accounts back to Unionbank.
  4. Submit the completed form to the GSIS personnel assigned in the eCard enrollment area.
  5. GSIS will notify you via text message where and when you can get your eCard / UMID or Temporary Card.
  6. Receiving and activating your card:
    1. For eCard/ UMID Card (both issued by UBP and LANDBANK) holders. Once you receive your card, go to any GSIS Wireless Activated Processing System (G-W@PS) kiosk located at the nearest provincial capitol, city hall, selected municipal offices, government offices, GSIS branch offices and Robinsons Malls. Place your eCard on the G-W@PS Kiosk card slot. Begin the activation process by touching the screen and selecting any of pre-registered fingers. Put the selected finger very lightly on the fingerprint scanner to activate your eCard/UMID Card. In cases where the selected finger fails to get scanned, simply select another pre-registered finger.
    2. For temporary eCard holders. Activation is done manually and in person. Visit a GSIS Office and present two valid government IDs to the frontline lobby officer for the manual activation of your eCard.
    3. If you transferred from UBP to LANDBANK. Wait for a text from Land Bank informing you that your eCard is now ready for pick up. Present two valid government IDs to claim your LANDBANK-issued eCard. The card is activated immediately upon receipt.
    4. For pensioners based abroad:
      1. After you have received your eCard Plus/UMID Card, set up a Skype meeting with GSIS by sending an email in to [email protected]. Note: Use Manila date and time for the meeting.
      2. Wait for the email confirmation of the preferred schedule from [email protected].
      3. On the date of your appointment please have your ecard Plus and/or scanned/photo copy of ecard Plus/UMID and/or 2 Valid IDs or Passport ready. Also, make sure that your computer is positioned in a well-lit area.
      4. Log in to your Skype username/account at least 15minutes before your scheduled time. Wait for a call from GSIS using these following Skype username/account: gsis.global1 / gsis.global2 / gsis.global3 / gsis.global4 / gsis.global5 / gsis.global6.
      5. During the video call, an interview will be conducted by a GSIS Skype Interviewer to determine pensioner’s identity and eligibility. If found eligible, the card will activated.

Application is done in person by going to a Government Service Insurance System (GSIS) office. When you apply make sure to bring two valid government-issued IDs (example: LTO license, PRC license, passport, voter’s registration ID).

eCard Plus: This is what a GSIS membership ID Card is also called. It is an all-in-one card that allows you to transact with GSIS. It looks like a regular ATM Card and can be used as one. It can also be used as a debit card. You may present your eCard Plus as a hospitalization discount card, medicine discount card, and tuition discount card.
