Assam - Online Apply for Certificate of Identification (COI)(Form submission)


Apply Online via PSK ( Online form submission)

Step 1 : First one has to make sure that all the required documents are in hand to apply online. If you would like to know about the documents that are required for this procedure, please visit the following link : Required Documents to apply for COI (Via PSK)
Step 2 : Applicants must visit the official website of the Passport Seva Kendra to apply for the Certificate of Identification: Apply Online
Step 3 : If you are a existing user click the ‘Existing User Login’ Button which is highlighted in “Green” in the below image. If you are a new user Click on the ‘New User Registration’ button which is highlighted in “Red” in the below image.

Step 4 : Fill up all the details and click on the ‘Register’ button which is highlighted in “Red” in the below image.

Step 5 : Now enter your user ID and click “Submit” button which is highlighted in “Red” in the below image.

Step 6 : Once you successfully registered yourself, on the dashboard click on the ‘Existing user login’ button to log-in to your account which is highlighted in “Red” in the below image.

Step 7 : Now login using your Id and click “Continue” button which is highlighted in “Red” in the below image.

Step 8 : Now enter your User name, Password and characters displayed and click “Login” Button which is highlighted in “Red” in the below image.

Step 9 : Click on the ‘Apply for Identity Certificate’ on the dashboard which is highlighted in “Red” in the below image.

Step 10 : Now click on the ‘Click here to fill the application form online’ button which is highlighted in “Red” in the below image.

Step 11 : Fill up all the details in the form, upload a copy of all the required documents, and submit.
Step 12 : Next, the payment gateway will open where you will need to make the payment.
Step 13 : Once the payment is successful, you will receive a confirmation SMS on your registered mobile number.
Step 14 : You will receive another SMS on the registered mobile number after the authorities verify the submitted form and the documents, stating the date and time on which you need to visit the PSK.
Step 15 : On receiving the SMS, visit PSK on the asked date and time with the original documents.
Step 16 : On the day of reporting, the concerned authorities will verify all the original documents.
Step 17 : Once all the above procedures are successfully completed, you will be given a receipt. Keep the receipt safe and wait till you receive any further information from the PSK.
Step 18 : You will receive an SMS on your registered mobile number, once your application is processed and the certificate is printed and ready for dispatch.
Note:
Step 1: Certificate of Identity is subject to clearance from the Ministry of External Affairs (Consular, Passport, and Visa (CPV) PV-III division), Government of India.
Step 2 : Clearance by State Government (Department of Home Affairs/Police)/FRRO is also required for issuance of No Objection to Return to India (NORI) certificate to be stamped on the Identity Certificate.
Step 3 : Application of Certificate of Identification through Passport Seva App, ‘mPassportSeva’ is currently unavailable.
