Next of Kin means the closest living blood relative or relatives. This certificate is required to prove that the applicant has a blood relation with that died person legally.

Apply In-Person:

  1. In order to apply for Next of Kin Certificate, the applicant has to approach their respective Deputy Commissioner/ Sub-Divisional Officer(Civil).
  2. Please go to their Respective DC Office/Circle Office/CSC Offices as appropriate.
  3. Link for the office contact details: District wise please select the respective district name and reach the portal. In the respective district page, user can navigate “Contact us” or “Know the department” or “Administration” menu options to get the contact details. AND Link2
  4. Go to the respective office. Consult with the designated authority about the applying procedure for this certificate. Collect the application form or write on a plain A4 sheet as advised.

  6. The application for Next of Kin(s) Certificate shall be submitted in standard format by the next of Kin(s) of the deceased person.
  7. Complete the application form. Attach the entire required document as per our “Required Documents” section and submit it to the designated receiving authority.
  8. After receiving the application, authorities will check the submitted application and document set. If all are in place the application will be accepted with applicable fees as appropriate.
  9. Authorities will make entries about this application in the manual record or electronically using computer.*Acknowledgement receipt will be given against every form received. So, collect it and please keep it safe for future reference.
  10. Applicant will be informed of the probable date of collecting the certificate.
  11. Designated authority will forward the application to the concerned Circle Officer / designate authority for his report.
  12. The Circle Officer / designated authority will verify the applicant’s details. A report will be created after verifying the details of the family members of the deceased person through field verification.
  13. This report will be sent back to the issuing authority.
  14. Based on the outcome of the above report and other internal processing the issuing authority will make a decision.
  15. The status, viz. Approval or Rejection if any will be conveyed to the applicant through an SMS.
  16. If approved, the certificate will be hand delivered or sent through post to the address mentioned while applying or issued as per applicable norms.


Note :
  • Certificate will be despatched or handed over directly as per the request made while applying by the applicant. Mostly handover in office will be in practice to get counter signature from the applicant for the delivered certificate.
  • Applicant may be asked to apply with the specific format as advised by the respective benefit providing government departments


Through CSC

  3. Application can be obtained from the authorities. Please pay for the application form as authorities quote.
  4. Submit the completed application along with required documents to the operator in the centre.
  5. Operator will check the submitted details and start to process online using the computer.
  6. Applicant will get the transaction ID for the application made. Please keep it safe for future reference.
  7. This application will be processed by the respective department as per how it will be processed in person.
  8. Applicant will get notification about the status of their application on their registered mobile number that was provided while applying.
  9. Applicant has to go and meet the respective authorities upon notification with documents in original as instructed to get the certificate as per notified date.

  2. Please select “Sign In” on top of the page to reach next page.
  3. Please complete the Sign in details as applicable at the prompted space and click “Sign In” button to sign in.
    • If you are a new user, use the “Create Account” option to get create account page. Please fill in the required details as per prompt and click “Save” button to register and then Sign In using the credentials.
  4. Once sign in applicant will reach dashboard. Here from the e-forms tab select “NEXT OF KIN CERTIFICATE“ from the dropdown list and click “Go” button.
  5. Applicant will reach online filing page. Please follow the page prompt as appropriate to complete the mandatory details as per page prompt.
  6. Please upload required documents at the prompted space as appropriate.
  7. Once above steps are completed, the user should make the required payment for the online application as required using the applicable payment gateway.
  8. After payment, a computer generated acknowledgement will be provided. Please take a printout. Keep it safe for future reference.
  9. This application will be processed by the respective department as per how it will be processed in person.
  10. Applicant will get notification about the status on his registered mobile number provided while applying.

  • Applicant should be Wife/ Husband/ Mother/ Father/ Daughter/ Son/ Brother/ Sister /Uncle /Aunty /Nephew /Niece of the deceased person.



FeesEdit

  • Please pay as authorities quote while applying.
  • Please pay the applicable Service Charge, Printing Charge and Scanning Charge
  • For online, please pay as per prompt while processing.
  • Following fee need to be paid.
    • Service Charge: Rs.30/-
    • Printing Charge: Rs 10/- (Not required for Senior Citizen)
    • Scanning Charge: Rs 5/ per page (Not required for Senior Citizen)



ValidityEdit

  • This certificate is valid for entire lifetime.


Note: In some cases the certificate shall be issued for the specific requested purpose. This may be in view of gaining benefit by the next-of-kin of the state government servant, who die while discharging their duties.
