Apply In Person

  1. To Apply for an Affidavit of Birth, the applicant has to approach the U.S Department of States link
  2. The applicant has to download the application form from the following link
  3. The applicant has to enter all the details in the application form.
  4. Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  6. Submit all the necessary documents along with the completed form to the State court.
  7. The detailed address is mentioned in the “Office Locations & Contacts” section of this page.
  8. The fee for this procedure has to be paid as per the authority’s request.
  9. After receiving the application, the concerned authority will check the submitted application and document set. If all are in place, the application will be accepted.
  10. Then the application will be processed further. All the updates on the application status will be sent via SMS to the applicant’s mobile number.
  11. Once the process gets completed, the applicant gets the notification through mail.
  12. The applicant can download the affidavit birth certificate through email.
  13. The processing time for this procedure is up to 15 – 30 days.

Difference between a birth certificate and an Affidavit of Birth

  • The birth certificate is issued often by a hospital, at the moment of (or very shortly after) the live birth of a human baby. The birth certificate is a vital proof record. It holds more authority than any Affidavit of Birth.
  • In the United States, someone who does not possess a birth certificate can use an Affidavit of Birth to apply for a new one.
