Procedure

Apply In-Person:

1. To Apply for Fingerprint Card, the applicant has to approach the federal bureau of investigation Link.
2. The applicant has to download the form from the given following Link. Applicant should make sure to carry all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
3. Applicant has to fill in all the necessary details in the form and attach the photocopies of the documents that are needed along with the application form before submitting to the local police stations - U.S. Embassy and Consulates General.
4. The fee of this procedure is mentioned under the “Fees” section of this page.
5. Concerned authorities will look into the application form and submitted documents before providing an approval.
6. The card will be provided within 16-18 weeks by the local enforcement agency.
7. If the card has not been provided within 16-18 weeks, applicant can contact FBI directly to know status of their application.

Instructions

• Prints should be checked with the appropriate state identification bureau in general, and only those fingerprints should be submitted for FBI search if no disqualifying record has been found locally.
• In the space "employer and address," the identity of private contractors should be displayed.
• The name of the agency submitting the fingerprint card to the FBI is the contributor.
• Universal control number should always be furnished in the correct space if known.
• Other armed forces no. [FP], alien registration no. (AR), port security card no. (PS), selective service no. (SS) veterans' administrations claim no (VA).

Need for the Document

• The card is used for:
  • Law enforcement agencies in fingerprinting applicants for law enforcement positions.
  • state and local government officers for employment, licensing, and permits, as authorized by state statutes and approved by the attorney general of the united states unless explicitly based on applicable state statutes, local and county ordinances do not satisfy this requirement.
  • Us government agencies and other entities required by federal law.
  • Officers of federally chartered or insured banking institutions who are responsible for promoting or maintaining their security.

Information which might help

• On fingerprint cards, do not use highlighters.
• In 'Leave Blank' boxes, do not input any data or labels.
• Ensure that fingerprint impressions are thoroughly rolled from one nail to the next.
• As certain that the fingerprint imprints are in the correct order.
• Make sure to make a note of any missing fingerprint impressions (i.e. amputation).
• Per fingerprint imprint block, no more than two retabs should be used.
• Make sure there are no stray markings in the fingerprint impression blocks.
