Procedure

1. Fill out a Commercial Driver License Application. You will need to provide your Social Security number. It will be used to verify your identity and to comply with federal and state child-support enforcement laws. It will not be used as your license number. You must show a valid Social Security card at the time of application.
2. Submit the form along with all other required documents like, proof that your presence in the United States is authorized under federal law. See CDL Documentation Requirements for a listing of acceptable proof.
3. You must be able to present two forms of identification. One form must have your photo, or you must present three forms of identification if no photo identification is available. See the full list of Identification Requirement. All forms of identification must be originals or copies certified by the issuing agency. MVD cannot accept photocopies or notarized copies of documents. All forms of identification must be in English.

Eligibility

For Commercial Driver License (Class A, B or C) you must be at least 18 years of age.

To qualify for an Arizona CDL, your state of domicile must be Arizona. The state of domicile is defined as the state of your true, fixed and permanent home and principal residence; it must be where you intend to return after being away.

Note: You have 30 days to change your CDL when you change your state of domicile to Arizona. Federal and state laws require that you surrender all previous license credentials.

For New Licenses or Transfer from Another State:

• Class A or B: $25.00
• Class C (must include a P endorsement or HME): $12.50
• Add $10.00 for each endorsement you require (hazmat, tanker, doubles/triples or passenger). Motorcycle endorsement is $7.00.

Duplicate: $12.00

Renewal

• Class A or B: $15.00
• Class C: $10.00
• HME: $10.00 (knowledge test required)

Required Road/Skills Tests:

• Class A or B vehicle: $25.00
• Class C vehicle: $12.50
• Passenger or school bus endorsement: $5.00 (when adding to an existing CDL)

Other Fees:

Notes:

• For revoked, disqualified, suspended or canceled licenses, you may be required to pay another application fee in addition to a reinstatement fee.
• Payment may be made by cash, check, travelers check or money order (payable to Motor Vehicle Division) for most fees; credit cards may be used at some offices. Returned check and reinstatement fees may not be paid by check.

Validity

Arizona issues an "extended" driver license that does not expire until age 65. However, your photo and vision screening will need to be updated every 12 years. Drivers age 60 and over will receive a 5-year license.

Instructions

• A commercial driver license (CDL) is required for drivers of heavyweight vehicles, 26,001 lbs or more gross vehicle weight (GVW), vehicles capable of carrying 16 or more passengers (including the driver), or vehicles required to be placarded for hazardous material.
• The minimum age for a commercial driver license is 21. If you are at least 18, you may apply for an intrastate commercial driver license that is valid only in Arizona.
• You are required to submit a copy of your DOT Physical Examination form when you apply for a commercial driver license, or renew an existing license. Incomplete or inaccurate information on the examination form may result in suspension or denial of the license.
• You must maintain your physical qualification with MVD and must submit a copy of your examination form every 24 months to the MVD Medical Review Program. Failure to do so may result in suspension of your commercial driving privilege.

Commercial Driver's Licenses (CDL's) are being issued by the Motor Vehicle Division of ADOT. Knowledge and road/skills tests are conducted at any CDL office. In addition, MVD works closely with the Medical Review Program, which examines and monitors the physical qualifications of commercial drivers.

Need for the Document

A driving license is an official document which states that a person may operate a motorized vehicle, such as a motorcycle, car, truck or a bus, on a public roadway. Also use as identification and proof of age.

The Commercial Motor Vehicle Safety Act of 1986 ensures that only qualified drivers receive and maintain a commercial driver's license (CDL) and removes unsafe and unqualified drivers from highways.

Commercial driver licenses (CDLs) are divided into three classes: Class A, B and C.

• Class A CDL: required to operate a combination vehicle (truck and trailer) if the gross combined weight rating (GCWR) is 26,001 or more pounds when the gross vehicle weight rating (GVWR) of the trailer, which is added to the GVWR of the power unit (the truck), is 10,001 pounds or more.
• Class B CDL: required to operate any vehicle with a GVWR of 26,001 pounds or more. A trailer may be towed if the GVWR of the trailer is 10,000 pounds or fewer.
• Class C CDL:' required to operate any vehicle with a GVWR of 26,000 pounds or fewer if the vehicle is required to be placarded to transport hazardous materials or if the vehicle transports 16 or more passengers, including the driver. An applicant must also have a passenger (P) or hazardous materials endorsement (HME) to obtain a Class C CDL.
