Apply In-Person
- To Apply for Modified Scooter (Ex-Servicemen), please visit the Office of the Department of Ex- Servicemen welfare, Sainik Board of their respective area. The address and contact details of the head office can be found in the following Link.
- Visit the Office and get the application form to apply for Modified Scooter (Ex- Servicemen) from the relevant department.
- Please fill the application form with the essential information and details and should be attested with the applicant’s signature.
- Please attach all the documents that are listed in the “Required Documents” section of this page.
- Then, it should be submitted to the relevant authority. Also, pay the required fees for the application process at the office as directed by the officials. Please collect the acknowledgement slip for the paid fee. Keep it safe for future reference.
- The application and attached documents will be sent for verification to check, if there is any discrepancy. The applicant will be called in person to verify all the documents.
- After all the processes are completed, the Zilla Sainik Board will approve the application and forward it to the Regional Sainik Board.
- The Regional Sainik Board recommends the application to the Kendriya Sainik Board.
- The financial assistance for procurement of mobility equipment for disabled Ex Servicemen scheme will be sanctioned for the eligible candidate to get modified scooter.
- Please check the “Eligibility” category to know about the eligibility criteria.
- This Welfare scheme grant application may take a minimum of 45 days to process.
- Once all the requirements and verifications are done, the amount will be transferred to the applicant’s bank account for Modified scooter or the applicant will be informed to collect the amount in person.
Apply Online:
- To apply online for Modified Scooter (Ex- Servicemen), please visit the following Link.
- Click on “Register” to register as a new user. If already registered, the applicant can click on “Login” with their Login credentials.
- Now under Welfare schemes, please select “Financial assistance for procurement of mobility equipment for disabled Ex Servicemen (all ranks)” and proceed with “Apply Online”.
- On the opened page, please fill the application form with appropriate details.
- Please scan and upload the necessary documents.
- After all the columns are filled appropriately, check once and please click on “Submit”.
- The application then reaches online to the respective Zilla Sainik Board.
- The Zilla Sainik Welfare staff verifies the application after giving an appointment to the applicant.
- The Zilla Sainik Board Officer recommends the case and forwards the same to Regional Sainik Board where Secretary Regional Sainik Board recommends the case, which finally reaches Kendriya Sainik Board.
- Once the application reaches KSB, the Staff and the Officer at KSB after due checks approve the application. Approved applications are then forwarded for payments via NEFT through Account’s section of KSB.
- After approval of the cases, the approved cases will be processed for payment via NEFT by the Accounts section of KSB.

- Complete Discharge Book/Document/Certificate.
- ESM Identity Card.
- Documentary evidence showing nature of activity in which disabled.
- Disability Certificate issued by Armed Forces Medical Authority, indicating nature of disability and recommended procurement of mobility equipment.
- Financial estimate for modified scooter from an authorized dealer indicating type, make and specifications of the mobility equipment.
- Details of Bank A/c No (in PNB/SBI only) and IFS Code.
- Copy of Aadhaar Card of the applicant ESM
- Payment receipt

EligibilityEdit
- Applicant must be ESM disabled after retirement from the service with disability of 50% or more, and not covered under similar scheme of the Army/Navy/Air Force.
- The applicant should be capable of personally utilizing the mobility equipment so provided.
- The application should be recommended by respective ZSB.
- The applicant should not have obtained any financial grant for this purpose from State/Service.
