  • To apply for a Change of Name Certificate, contact a registry agent who will be able to assist in placing the order for the certificate.

Receiving of the CertificateEdit

Documents are mailed to the address of the person who is making the application. It is advisable to contact a Registry Agent or Registry Connect for special arrangements and/or rush services options.

EligibilityEdit

  • a) The person whose name was changed
  • b) The person who applied for the change of name
  • c) A guardian or trustee of a person whose name was changed. A copy of the guardianship or trusteeship document must be attached to the application.
  • d) A person who is a designated agent for a person described in a), b), c), g), or h). Consent and the statutory declaration in the application must be completed.
  • e) A person who holds power of attorney to act on behalf of the person described in a). A copy of the legal document must be attached to the application.

  • f) A person with an order from the court. A copy of the order must be attached to the application

When the person whose name was changed is a minor:

  • g) The parent of the person whose name was changed as established by a Registration of Birth or court order. A copy of the applicable court order must be attached to the application.
  • h) A guardian, trustee, or person with power of attorney, for a parent of the person whose name was changed. A copy of the legal document must be attached to the application.

When the person whose name was changed is deceased:

  • i) The executor of a persons estate. Death certificate and a copy of the will identifying the executor must be attached to the application.

Anyone may apply for a Legal Change of Name Search Letter.

In addition to the government certificate fee, registry agents are authorized to charge a service fee.

Processing TimeEdit

Once a registry agent submits a request or the Vital Statistics office receives an application, it is usually processed within two to three business days provided there is no additional information required.

  • Full name of the person making the application
  • The relationship between the person making the application and the person whose document is being changed and proof if applicable
  • Full return address of the applicant
  • Purpose for your request
  • Signature of the person making the application
  • Date the application is being made
  • Phone number or contact phone number of the person making the application
  • Details pertaining to the legal change of name (e.g. the old name and new names of the person who's name was changed, their sex, approximate year the change of name was registered)

Note: If all the required information is not supplied, your request may be delayed.
