Procedure

Apply In-Person:

1. To apply for Certificate of Revival of Co-operatives in person, the applicant has to visit the nearest Service Alberta office.
2. Alberta Supports or Alberta Works Centre around all locations in Alberta can be found in the following contact link
3. The applicant has to visit the office with respect to their location and obtain the application form for Certificate of Revival of Co-operatives from the relevant department.
4. Make sure that all the documents listed in the “Required Documents” section of this page, are available.
5. Then, the applicant should complete the application form with required details and information under each section.
6. After completing, all the required documents should be attached with the completed form.
7. Then, submit the application package to the relevant authority at the office. The application package can also be submitted through email or mail.

9. The applicant has to pay 250 dollars for the process through any of the available payment modes.

11. The review may take 10 days and on completion of the review, the office will inform the applicant through phone number or email to collect the Certificate of Revival.

1. To apply for Certificate of Revival of Co-operatives through mail at Corporations Canada, the applicant has to forward an application to mail address of the Corporations Canada.

3. First, the applicant has to request an application form for Certificate of Revival of Co-operatives by completing and submitting the following Request form

• Any Cooperative in Alberta, that has been dissolved, is eligible for the Certificate.
• Any authorized person can act as an applicant and the applicant must have the Corporation number while applying.

• The applicant has to pay 250 dollars for the process through any of the available payment modes.
