Procedure

Sending request via mail

1. Download Application form online. You may not request two different types of certificates using the same form.
2. Prepare your payment, which may be in the form of a check or money order payable to "State Board of Health".
3. Send the completed form along with your payment to:

Obtaining certificate in person

1. Go to any county health department in the State of Alabama to obtain a certified copy of an Alabama divorce certificate. Most divorce certificates can be issued while you wait.
2. Complete a request form and pay all necessary fees.

Eligibility

By Alabama law, divorce certificates are unrestricted. They may be requested by anyone who can provide enough information to search for the record and submit payment of the proper fee.

Fees

• Fee to search for a divorce certificate: $15.00 (includes one certified copy of the divorce record or a "Certificate of Failure to Find")
• Additional copies: $6.00 each

• Your check or money order must be payable to "State Board of Health". Please do not send cash. Fees are non refundable.

The Alabama Center for Health Statistics keeps divorce records filed in Alabama beginning 1950. For divorces prior to 1950, the certificate must be obtained from the circuit court in the county where the divorce was granted.
