Procedure

Apply In-Person:

1. To Obtain Disposition of Materials Certificate, the applicant has to approach the Nuclear Regulatory Commission (NRC) Contact details are mentioned in “Office Locations & Contacts” of this page. Or Use the following Contact Link
2. An application form is available at the following Link
3. The applicant should make sure to carry all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
4. Applicant has to fill in all the necessary details in the form and attach the photocopies of the documents that are needed along with the application form before submitting to the Nuclear Regulatory Commission (NRC).
5. The application is reviewed by the NRC using the leak test data for this licensing application.
6. A Certificate is issued by the NRC after approving the application.
7. The fee for this procedure has been mentioned under the “Fees” section of this page.
8. Complying with this required collection request is estimated to take 30 minutes per answer.
9. The processing time will be 3 days to 1 week.
10. For Further details of receiving the certificate will be intimate by the NRC regional office.

Instructions

• Two leak tests will be performed every six months.

Required Information

The information needed to be filled in the application form are listed below:

• License status
• Disposal of radioactive material
• Surveys performed and reported

Need for the Document

• The licensee, or any person acting on the licensee's behalf, declares that no radioactive materials have ever been purchased or owned by the licensee under this licence.
• Any operations authorised by this licence have ended, and all radioactive materials obtained and/or possessed by the licensee under the above-mentioned licence number have been disposed of in the transfer of radioactive materials to the licensee and disposal of radioactive materials.

Information which might help

• The specific radioactive material transfer actions should be described by licensees.
• If radioactive wastes were developed during the termination of this licence, the licensee must indicate how they were disposed of, including low-level radioactive waste, mixed trash, waste larger than Class C, and sealed sources.

Other uses of the Document/Certificate

• The NRC uses this submission as part of its basis for releasing the equipment for unrestricted use.
