    NOTE: Enter the postal code or city, select the facility type and VA beneficiary service. Finally click the “Search” button to check the nearest office centre.
  2. The applicant has to download the application form from the following link
  3. Enter all the details in the application form.
  4. Attach all the necessary documents which is mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  6. Submit all the documents with the completed form to the nearest VA medical centre or clinic or get help through the state’s Department of Veterans Affairs.
  7. The fee for this health care certificate is free of cost.
  8. Once the verification gets done by the officer, the applicant receives the notification from the office through mail.
  9. The applicant receives the health care card by visiting nearest office, also the applicant can ask the further details about doctor visit.
  10. The processing time for this procedure is within one week.

EligibilityEdit

The applicant may be eligible to enrol in VA health care if all of these statements are true:

  • The applicant served in the active military, naval, or air service (including being called up from the National Guard or Reserve by a federal order), and
  • The applicant didn’t receive a dishonourable discharge, and
  • The applicant should meet at least one of the service requirements for enrolment


The applicant must meet at least one of these service requirements:

  • The applicant served at least 24 months in a row without a break (called continuous), or for your full active-duty period, or
  • The applicant was discharged for a service-connected disability, or
  • He/she were discharged for a hardship or early out,
  • The applicant served before September 7, 1980
