Apply In-Person:
- To apply for mother or father insurance benefits (SSA), the applicant has to approach the U.S Social Security Administration link
- NOTE: Enter the Zip Code and click the “Locate” button to check the regional address.
- The applicant has to get the form from the respective department or use the following link to download the form link
- The Applicant should make sure to carry all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
- Applicant has to fill in all the necessary details in the form and attach the photocopies of the documents that are needed along with the application form before submitting.
- Once completed, the applicant should submit the Form to U.S Social Security Administration department office.
- Fee for this procedure is free of cost.
- Concerned authorities will look into the application form and the document.
- Once the process is done, the applicant can get the document by visiting the office within the week.

- To apply for mother or father insurance benefits (SSA), the applicant can also do by call 1-800-772-1213 (TTY 1-800-325-0778).
- Give the information to the respective officer.

EligibilityEdit
To be eligible for this benefit program, the applicant must meet all of the following requirements:
- Be unmarried.
- Meet the marriage duration requirement.
- Have in your care the deceased's child who is under age 16 or disabled and entitled to child's insurance benefits (for a surviving divorced mother or father the child must be their natural or legally adopted child).
- Not be entitled to a higher Social Security retirement benefit on your own record.
- Not be entitled to a widow's or widower's benefit.
- Be finally divorced from the deceased worker (Surviving Divorced Spouse Only).
- Note: Surviving Divorced Mother’s and Father’s do not have to meet the 10-year marriage duration.
