Apply In-Person:

1. To apply for mother or father insurance benefits (SSA), the applicant has to approach the U.S Social Security Administration link

2. The applicant has to get the form from the respective department or use the following link to download the form link
3. The Applicant should make sure to carry all the documents that are mentioned under the “Required Documents” section of this page (Both photocopies and originals).
4. Applicant has to fill in all the necessary details in the form and attach the photocopies of the documents that are needed along with the application form before submitting.
5. Once completed, the applicant should submit the Form to U.S Social Security Administration department office.
6. Fee for this procedure is free of cost.
7. Concerned authorities will look into the application form and the document.
8. Once the process is done, the applicant can get the document by visiting the office within the week.

Eligibility

To be eligible for this benefit program, the applicant must meet all of the following requirements:

• Be unmarried.
• Meet the marriage duration requirement.
• Have in your care the deceased's child who is under age 16 or disabled and entitled to child's insurance benefits (for a surviving divorced mother or father the child must be their natural or legally adopted child).
• Not be entitled to a higher Social Security retirement benefit on your own record.
• Not be entitled to a widow's or widower's benefit.
• Be finally divorced from the deceased worker (Surviving Divorced Spouse Only).

Note: Surviving Divorced Mother’s and Father’s do not have to meet the 10-year marriage duration.
