InstructionsEdit
Within 48 hours of discovering a theft or loss of a firearm from the FFL's inventory (including any firearm transferred from the FFL's inventory to a personal collection and held as a personal firearm for at least one year), or from the collection of a licensed collector, each Federal Firearms Licensee (FFL) must report the theft or loss.

Need for the DocumentEdit
Federal Firearms Licensee(FFLs) must fill out ATF Form to record such thefts or losses, and the theft or loss must be reported to the relevant local authorities.
Information which might helpEdit
- Because there is no national registration system, ATF cannot assist private persons in locating serial numbers.
- One of the following choices might be helpful:
- Make contact with the guns dealer where the applicant bought the weapon.
- If the guns dealer is no longer in business and the applicant have a question about a stolen weapon, contact the local police station.
- If the conditions are tied to a legitimate criminal investigation, it's probable they'll submit a Records Search Request to the National Tracing Center.
- If their state has a guns registration office, contact them.
Other uses of the Document/CertificateEdit
- This form should be used to notify the ATF Stolen Firearms Program in writing of any firearms thefts or losses.
- The applicant will be given an ATF Issued Incident Number to record at a later time.
