Apply Online

  1. To Apply for Contractor ID Request, use the following link
  2. To register this, the applicant has to click the “New E-Gov users register here”.
  3. In the following link the applicant has to enter all the details to register.
  4. After entering all the information, the applicant has to click “Register”.
  5. Once completed, the applicant has to use the first link to login for applying.
  6. Enter the mail ID password, and click the “Sign – In” button.
  7. Now, the applicant has to enter all the details in the application form.
  8. Make sure that the applicant has to upload all the necessary documents that are mentioned in the “Required Documents” section of this page.
  9. And also upload the instructor biometric signature.
  10. Pay the fee on the online mode.
  11. Once completed, click the “Submit” button.
  12. Once the verification process gets done by the officer, the applicant gets the notification from the department.
  13. The applicant gets the certificate through mail.
  14. The processing time for this procedure is within 15 – 30 days.


Apply by Fax

  1. To Apply for Contractor ID Request, the applicant has to approach the U.S. DEPARTMENT OF LABOR Mine Safety and Health Administration link
  2. The applicant has to download the form from the following link
  3. The applicant has to fill in all the necessary information in the application form. (Incomplete application form are rejected)
  4. Make sure that the applicant has to attach all the necessary documents that are mentioned in the “Required Documents” section of this page.
  5. The applicant has to attach the photocopies of the required documents along with the application form while submitting.
  6. Submit all the documents with the completed form to the office fax number - (303) 231-5515.
  7. The fee for this procedure has to be paid as per the authority’s request.
  8. Once the verification process gets done by the officer, the applicant gets the notification from the department.
  9. The applicant gets the contractor ID through mail.
  10. The processing time for this procedure is within 15 – 30 days.

  • Small contractors performing work for the federal government may need to obtain a government access card/ ID number. The card allows contractors who will be working in sensitive areas to have access to secured federal facilities, computer systems and networks.
  • The cards are processed through the Identity, Credential and Access Management Division, which does background checks to verify the personal identities of individual contractors to ensure terrorist or other unauthorized personnel cannot gain access to secured areas and materials.
