Apply In-Person:

1. Application form for renewal of registering as a veterinary practitioner in Uttar Pradesh is available in Uttar Pradesh State Veterinary Council.

3. After getting the application form, the applicant has to fill in all the necessary details.
4. Applicant should make sure to carry the originals and photocopies of the documents that are listed under the “Required Documents” section of this page.
5. Applicant has to attach the photocopies of the documents along with the application form while submitting it at the office.
6. Fee must be paid as per the authority’s request.
7. Renewal of registration can be done within 15 days. If the renewal has not been done within a month, then, the applicant has to visit the council to know the status of their application.

1. Application form for renewal of a veterinary practitioner in Uttar Pradesh is available on the following link : Link
2. Existing users, can login directly by using their “E-mail ID/Mobile Number”, “Password” and “Captcha”.
3. Finally, they should click on the “Login” button.
4. New users can login by clicking on the “If don’t have login credentials” button and then enter their “Registration Number”. Then, click on the “Generate OTP” button.
5. OTP will be sent to the respective mobile number or E-mail ID.
6. Applicant can login by entering the OTP.
7. Applicant should fill in all the necessary details in the registration form.
8. If necessary, the applicant should attach the digital copies of the documents that are listed under the “Required Documents” section of this page.
9. Fee for this procedure can be paid online.
10. After completing all the above, the applicant can click on the “Submit” button.
11. Renewal of registration can be done within 15 days. If the renewal has not been done within a month, then the applicant can check the status of their application online or by visiting the state veterinary council.
