1. To obtain a replacement passport, applicants must go to a State Migration Service office (address to those offices are provided below).
2. Complete an application and submit it together with these following documents:
   1. Three passport photos
   2. Statement of Loss or Theft
3. Pay the application fee.

If you are outside Ukraine

1. For applicants outside Ukraine, you will need to file a report regarding the incident with the authorized bodies in the country you are currently staying.
2. Once you have the report, go to the nearest Diplomatic Mission of Ukraine and complete an application to obtain a replacement passport. You may use the link provided under "Office Locations & Contacts" to get in touch with the said office.
3. Submit the completed application together with the certificate of loss and other required documents.
4. Pay the application fee.
