1. To replace a lost or damaged Gun / Firearms License you will need to submit an application to the Arms License Section of the Ministry of Interior.
2. A link is provided under "Documents to Use" section which you can use to download the form.
3. Submit the application to Section officer Arm MOI with these documents:
   1. FIR/Police Report attested by SDPO.
   2. Attested Copy of CNIC.
   3. Entry record of licence/weapon being maintained by DCO concerned
   4. Affidavit PKR 100
   5. Note: Arms License Section has the right to reject the attached documents if not found up to the required standards.

• Application in the name of section officer Arm MOI.
• FIR/Police Report attested by SDPO concerned.
• Copy of CNIC attested by grade 18 officer's.
• Entry record of licence/weapon being maintained by DCO concerned
• PKR 100 Affidavit attested by public nortry.
