2. The applicant will need to complete an application form. Click on the following link to download the application form Link
3. Submit the application form to one of the addresses mentioned on the last page of the application form.
4. The applicant will need to pay a fee, find further information under the “Fees” section of this page.
5. The applicant will receive their driver’s license extract 3 days after the submission of the application, it can be collected from the counter after fee payment.

Apply Online

1. To obtain a driver’s license extract, the applicant can use MyGov NL. Here is the contact Link
2. If the applicant has an account, log in. If the applicant does not have an account, follow these steps:
   • On the log in page, click on the “Create an account” option.
   • Enter all the relevant information such as name, address, telephone number and so on.
   • Agree to the terms and conditions and click on “Next”.
   • The applicant will need to confirm the email by clicking on the verification link sent to the applicant’s email ID.
   • Once all the details have been entered, click on “Submit” to create an account.
3. Once logged in, the applicant will need to click on the “Driver and vehicle” option.
4. The applicant will need to consent to the terms of use and click on “Next”.
5. Enter all the relevant information such as driver's license number, last name, expiry date and postal code. Click on submit.
6. The applicant will be able to view their driver's license and can place an order for their driver’s license abstract.
7. The applicant will need to pay a fee, find further information under the “Fees” section of this page.
8. The applicant can download and print their extract 3 days after the submission of the application.
